PERFORMANCE WORK STATEMENT
To
Transport and Dispose of Advanced Wastewater Treatment
Facility Residual (Sludge)

PERFORMANCE WORK STATEMENT
WORK INCLUDED: The Service provider shall furnish all vehicles, trailers, containers, tools, equipment, parts, materials, supplies and labor for the loading, handling, transportation, unloading and disposal of residual (sludge) generated at Hurlburt Field’s Advanced Wastewater Treatment
Facility (HFAWWTF)(Building 91500). The vehicles shall be in working conditions to prevent the spillage of vehicle fluids and the containers shall be structurally sound to prevent the spillage of residuals (sludge) onto concrete/asphalt area. The containers for residual collection shall meet industry standards and shall be a minimum 25 cubic yard container (current working containers
20’L x 8’W x 4’D).
JOB DESCRIPTION: The work to be performed includes the following principal features:
a. Contractor shall transport and dispose of residuals (sludge) in accordance with the
Environmental Protection Agency Title 40, Code of Federal Regulations (CFR), Protection of Environment, 1993, Part 503, “Standards for the Use and Disposal of Sewage Sludge,” revised as of April 9, 2007 and effective on April 25, and the Florida Department of
Environmental Protection (FDEP) Standard for Treatment of Domestic Wastewater
Residuals, 62-640, F.A.C.
b. Contractor shall achieve vector attraction reduction, as outlined in the vector attraction reduction requirements, in Title 40 CFR Part 503, section 503.33(b) (10) (Incorporation) of, revised as of October 25, 1995 and 62-640.600(1)(b), F.A.C.
c. Contractor shall ensure land application of residuals (sludge) is in accordance with conditions of the approved Agricultural Use Plan(s) on file with the FDEP and the requirements of 62-640 F.A.C.
d. Contractor shall provide forms and related documentation and information to
HFAWWTF Supervisor (by mail: 1 SOCES/CEOIB, 415 Independence Road, Hurlburt
Field, FL, 32544, Attn: Wastewater Facility 91500 or fax 850.881.1512) in order to facilitate the reporting requirements of 62-640 F.A.C.
PERFORMANCE TIME
a. When requested, service provider shall deliver an empty container within 36 hours of notification.
b. When notified, service provider shall collect full container within 48 hours.
INSPECTION
a. Personnel of HFAWWTF shall serve as the on-site representative of the Contracting
Officer. Call 850.881.1511 for questions.
