Statement of Work Raised Flooring System
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide and install a Raised Flooring System and carpet on the second floor of the East Wing of Building 90207 on Hurlburt Field, FL.
1.1. BASIC SERVICES.
1.1.1. The contractor shall remove and dispose of old carpet and subflooring covering a 5,120 square foot area. The existing subfloor is Access Floor Systems bare concrete filled panels, size 24”x24”. The carpet and subflooring shall be disposed of by the contractor at an off base location.
1.1.2. The contractor shall provide new bare concrete filled replacement panels (Access Floor Systems Part Number AFP351 or equal) to cover the 5,120 square foot area. Replacement panels shall be compatible with the existing Access Floor Systems tracking system. The contractor shall cut 2 inch access points into the panels and fill hole with grommets to allow power cables to run through them unobstructed. Approximately 142 2-inch access points are required.
1.1.3 Flooring shall be installed in accordance with the manufacturer’s specifications, ADA federal regulations and in accordance with user/installation manuals as to not void any warranties.
1.1.4 The contractor shall provide 5,120 square foot of carpet. Carpet must be purchased from one of the four manufacturers that hold Tier 1 contracts under the AF Carpet program. Pricing for the carpet shall not exceed the established Not to Exceed pricing in the Tier 1 Contracts. Tier 1 contracts were awarded to Shaw, J+J Flooring Group, Mohawk, and CMA-Milliken. Product proposed must be one of the following offer under the AF Carpet program: Homeroom V 3.010351 Scholarship from Shaw; Impulse III Modular, Tell a Tale – 1811, Jump Right In from J+J Flooring; In the Loop, Group Earth-7869 from Mohawk or Landmark “Vestige”; VES121 Linseed from CMA-Milliken. POCs for these Tier 1 contracts are provided below.

1.1.5. Carpet shall be attached to the subflooring at the factory. Installation and application of carpet and adhesives shall be per manufacture specification.
1.1.6. The contractor shall move all furniture for product installation. After installation, the contractor shall return furniture to original location.
1.1.7. Prior to ordering product, the contractor shall perform a site survey with the Government to verify measurements, requirements, select carpet colors and finalize requirement.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. There will be no Government Furnished Property or Services provided for this effort.
2.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: MON-FRI 0700 to 1600, except for federal holidays. The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government.
2.2. Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
2.3 The installation shall not last more than 3 calendar days from start of the project. The contractor shall uncrate/unpack all new items and perform required assembly in accordance with the manufacturer’s instructions. Contractor shall remove and dispose of packing material at an off base location.
3. SPECIAL QUALIFICATIONS. The contractor shall be an authorized and/ or certified as a distributor/reseller of the project being offered as well as any subcontractors.
4. BASE ACCESS
4.1 Contractor, Employee Base Pass and Identification, Special Clearances, and Vehicle Passes. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation. The contractor shall submit the employee listing the Contracting Officer no less than 3 days prior to the performance start date and at least one day prior as changes occur. Employees with active warrants or felonies will not be granted access to the base.
4.2 The Contractor Employee List. This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center.
4.3 Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification. If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.
4.4 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
4.5 WEAPONS, FIREARMS AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field, FL.
4.6 DRIVING LICENSE/ON-BASE REQUIREMENTS. The contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Hurlburt Field, Florida.
