STATEMENT OF WORK
BUILDING P40 FURNITURE RECONFIGURATION
SCOTT AFB IL
3 April 2019
I. SCOPE
HQ AMC/SG requires systems furniture to be reconfigured to accommodate reorganization.
II. DESCRIPTION
a. Contractor will account and plan to install any existing panels, trim, cubicle framework, overhead cabinets etc…located in Bldg P-40E prior to purchasing new.
This will be coordinated and accomplished with the Government Project Manager.
b. Remove data and power cabling, as needed, from 8 workstations on the west side of room 123
c. Dismantle 8 (6’x6’x6’) “AMC standard” cubicles, and their 72” double-overhead cabinets
d. Re-install 8 workstations as shown in Attachment 1 (USE POE PROVIDED
ORIGINAL DESIGN) using ALL existing parts/pieces and newly purchased low wall workstation divider walls. Re-Run three (3) lines of existing CAT 6 cable from recently removed workstations. Additional parts/pieces should be minimal.
e. Contractor and Government Project Manager will locate 8 existing side-by-side single
36” overheads within the directorate for use/repurpose and install in new design for room 123. All side-by-side single 36” overhead cabinets will be replaced with the 72” double-overheads uninstalled from room 123 as a repurpose.
f. Contractor will re-assess workstations in room 123 along the exterior wall and remove agreed upon number of 6’ paneling blocking the window.
III. DESIGN SERVICES
a. Contractor shall be able to provide design services, including space planning, site verification, material selection and programming as necessary for this project.
b. Contractor shall define and furnish necessary hardware, software, and all other equipment necessary to accomplish design tasks. Standard equipment in addition to hardware and software shall include, but not necessarily be limited to: reproduction machines, plotters, paper, pencils, fax machines, and office furniture.
c. Contractor shall be responsible for collection of information relevant to design of a space. Examples could include personnel interviews, questionnaires and walk-through inspections of facility.
d. Services shall include site evaluation to ensure proper fit of new furniture within given space and all structural and other anomalies are taken into account. Contractor must field verify any provided layouts, AutoCAD drawing, and blueprints by performing a site visit.
e. Contractor is responsible for notifying Government Acceptor of all power requirements regarding location of communication, LAN and power junction boxes. User will be responsible for cabling, as well as, providing and locating those junction boxes within 4'-0" of ceiling penetration of power pole location provided by contractor.
f. Design shall ensure power poles do not protrude into main walkways.
g. Design shall ensure all outlets included in design are accessible to user, free and clear of obstacles.
h. Design shall not allow more than four cubicles to be served by a single circuit.
i. Design shall ensure no more than two duplex outlets (four plug in points) are put onto same circuit.
j. Design shall ensure no existing wall outlets or thermostats are covered or concealed by panels or other systems furniture components. Special circumstance may apply where Facility Manager may waive or make electrical accommodations if furniture must cover existing outlets.
k. All designs of furniture layouts, including original designs and any subsequent redesigns, must be accomplished by, qualified professional Interior Designers with a four year Interior Design degree and three years’ experience using software similar to that proposed for this contract. Resumes and letters of intent shall be provided for these employees.
l. Contractor shall provide additional drawings of furniture layouts, if requested by base organizations on a non-rewritable CD or DVD in both .dwg and .pdf formats unless otherwise specified in contract.
m. Hardware/software shall perform computerized inventory, computer aided design, automated take-off function, and report generation for all product purchased. Software for this contract shall be compatible software for AutoCAD and Microstation. Software shall be fully compatible with Microsoft Windows, most current version.
n. Design service shall include an initial complete design with up to two additional revisions prior to final approval.
o. Designs of each location shall include a basic component floor plan sheet in which all systems furniture and workstations are tagged: to include graphic symbols for all major hanging components (flipper cabinets, shelves, task lights, tack boards, work surfaces, drawers, etc.) as determined by government. This design sheet shall include station numbers and movable wall and associated product locations, in 1/4" = 1'-0" scale.
p. Designs of each location shall include a Panel and Hardware Layout sheet: to include all finish colors and fabrics, panel widths (30", 36", 42", etc.), in 1/4" = 1'0" scale, in addition to display hardware, keys and locksets shall be included.
q. Design sheet layouts of each location shall include a Furniture Power Distribution sheet in 1/4" = 1'0" scale to include: location of all new powered panels, powered components, receptacles, communication ports, power poles, powered raceways, base electrical feeds and building connection points.
IV. Installation and Removal Services
a. All dismantling, reconfiguration, and installation must be done by Haworth certified installers as to not negate the existing furniture warranty.
b. Contractor shall appoint an authorized representative who will be company's senior representation on site for all customer communication and enforce compliance of all requirements for project. This representative shall have a physical presence during any delivery and throughout installation unless agreed to by Government Acceptor and/or Government Project Manager. Resumes and letters of intent shall be provided for these employees.
c. Contractor representative shall meet with Government Acceptor and/or Government Project Manager to review installation drawings prior to installation.
d. Contractor shall be liable for any damage to government property caused by contractor or their representatives.
e. During installation, contractor shall protect all furniture and building materials, to include carpet, existing within space with industry approved protective coverings inclusive of craft paper, moving blankets and such type material. Contractor shall not use protective materials that will leave any type of residual print, outline or imaging on any furniture or building materials. Contractor shall not repair damage to finishes but will be responsible to pay for damages. Government will make repairs and back charge installation contractor.
f. Contractor shall uncrate/unpack all new items and perform required assembly in accordance with manufacturer instructions.
g. Services shall include inspection of product for shipping damage. If damage is found it shall be responsibility of contractor to notify authorized government representative and make all arrangements for replacement or repair of item(s) damaged. Damaged items are not to be installed unless otherwise directed by Government Acceptor.
h. Services shall include assembly of new product to conform to final Air Force approved design drawings.
i. Any furnishings attached to building shall be attached securely.
j. Contractor shall ensure all communication outlet faceplates are provided with new furniture and accommodate a standard RJ45 quad port or fiber-inserts that will be provided by local communications group or general contractor.
k. Services shall include removal of packing materials from installation.
l. All trash removed from worksite shall be disposed of in accordance with state laws, and packaging and other recyclable materials shall be recycled where ever possible.
m. Contractor shall maintain environmental controls when working in any Air Force facility to include, but not limited to, keeping doors shut and lights off when not in use and turning off vehicle engines when parked.
n. Light fixtures shall have appropriate lamps and be in working order.
o. Prior to acceptance, contractor shall clean all new furniture only with manufacturer's recommended cleaning agents/products.
p. Contractor shall provide all new components, completely assembled, installed, and operational, including keys in each lock set.
q. All furniture, equipment, and accessories shall be level, square, and in proper alignment with adjoining furniture. All drawers should operate smoothly and stay closed when not in use.
r. Contractor shall inspect to ensure new furniture is free of surface dirt and defects, installation is complete, and new furniture is ready for use.
V. Site Conditions
a. Standard site conditions shall be considered those that include:
(1) Free and clear hard surface access to receiving and installation location during regular working hours.
(2) Parking and off-loading area suitable for access by a semi-trailer combination or truck within 100 feet of building entrance closest to product installation area.
(3) Force Protection Condition level of base is Bravo. This requires dealer personnel to get base access in order to complete product installation.
(4) A staging area adequate to sort, stage and uncrate products.
(5) Includes electric power, working lights, heating and air conditioning.
Location is on first floor of building, but will require product to be carried up one or two flights of stairs. No elevator exists on P40 East wing.
VI. Additional Services Information
a. Contractor shall provide professional electrical and cabling services with install of new product. Any electrician provided by contractor shall be a licensed electrical contractor. Any cabling specialist provided by contractor shall be a Building Industry Consulting Service International, Inc. (BICSI) certified.
b. Contractor shall install all electrical connections to properly provide power to furniture per furniture manufacturer’s instructions and within National Electric Code guidelines.
c. Contractor shall install all data/voice communications cabling to provide proper communications capability for each cubicle.
(1) Include two data and one voice port at each workstation
(2) Label comm room/rack/patch panel/port on each drop
(3) Provide test results from patch panel to wall jack
(4) Install cabling on interior of furniture
(5) All cabling should be CAT6 or better
(6) All cabling installed to standard by EIA/TIA 568C
d. Any existing power poles/panels and/or service connections not used will be removed with wires terminated and hole in junction box plugged.
e. Contractor shall be responsible for collection of information relevant to redesign of a space. Examples could include personnel interviews, questionnaires and walk-through inspections of facility, field verifying any print, drawings, AutoCAD drawings, as well as, existing furniture database (if available).
VII Base/Installation Specifics
a. All commercial trucks must enter through Mascoutah Gate for inspection. Gate is located on south side of Scott AFB off of IL Route 161. In accordance with 375th Security Forces Squadron directives; base entry procedures must adhere to delivery vehicle process requirements mandated by AFI 31-113 Installation Perimeter Access Control. Once at Mascoutah Gate, driver must be issued a base pass for building P40, POC: Facility Manager, Mr. Thomas Gray at 229-0675. Drive should be able to show:
A current Vehicle Registration A current Proof of Insurance A valid and current Driver's License for driver A valid and current Driver's License or other State Identification for any passenger(s) in vehicle.
b. Gate Procedures:
(1) Once at Mascoutah Gate, be prepared to exit vehicle and allow for vehicle inspection. At this time, you may ask for directions to your final destination.
If final destination requires driver to leave Mascoutah Gate, and re-enter base through an alternate gate, pass received will be sufficient documentation for entry into both gates, within a pre-established time directed by Security Forces representative. Arrival at appropriate gate after designated allotted time will result in non-admittance to installation. This procedure will occur each time a commercial truck requests access to base.
(2) Current Bill of Lading (BOL) to get delivery driver and vehicle on base.
BOL must include AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).
(3) Prohibitions: Firearms may not be brought on base under any circumstances.
Individuals with criminal records are not allowed on base. Contractor personnel are prohibited from sponsoring guests onto installation.
VIII GENERAL INFORMATION:
a. Contractor Consent to Background Checks: The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified by Law Enforcement Agency Data System (LEADS) through the National Crime Information Center. LEADS checks will verify if a person is wanted by local, state, and federal agencies. All contractor and subcontractor personnel must consent to LEADS background checks. Contractor and subcontractor personnel who do not consent to an LEADS check will be denied access to the installation. Information required to conduct an LEADS check includes: full name, driver’s license number, and/or social security number, date of birth of the person entering the installation, and completion of a background check questionnaire. The contractor shall provide this information using the Scott AFB Form 21, Contractors Consent for Background Check, and shall submit it in conjunction with the contractor’s request for either base or vehicle passes. Completion of a successful LEADS check does not invalidate the requirement for an escort when contractor or subcontractor personnel are working within controlled or restricted areas. All identifying credentials will be compliant with the REAL ID Act.
Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Persons later found to be undocumented or illegal aliens will be remanded to the proper authorities. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this clause. Furthermore, nothing in this clause shall excuse the contractor from proceeding with the contract as required.
b. Badges: The contractor is required to provide identification badges for their employees.
All contractor personnel shall wear these badges while on duty on the government site.
Badges are required to identify the individual, company name, and be clearly and distinctly marked as contractor. Size, color, style, etc. are to be mutually agreed to by contractor and government. The contractor’s identification badge will not be used as an entry requirement for installation entry or into any government designated controlled or restricted area. All DBIDS cards/passes will be turned in to the Dixon Visitor Control Center upon expiration of pass and/or completion of contract.
c. Contractor Registration of Vehicles on Scott AFB: All contractor or contractor employees’ vehicles used for the performance of this contract shall comply with all local, state and federal regulations. Additionally any pass shall be surrendered to Security Forces upon demand to positively identify a person’s need to be on Scott AFB.
d. Access to Installation during Force Protection Conditions (FPCONs):
Contractors will be assigned a mission essential designation IAW requirements contained in SAFBI 31-101/Installation Security Instruction. Only the installation commander or the unit commander requesting contract support will assign the mission essential designation.
e. Property Protection: Property protection for facility where the contractors’ primary work center is located will be the responsibility of the local facility manager and local Government Security Manager, or their duly authorized representative IAW AFI 31-101, Integrated Defense and command/local directives. The contractor shall safeguard all government-owned equipment and materials in his/her possession or use.
f. Safeguarding Classified or Unclassified Information: The contractor shall meet Air
Force standards for storing, processing, and handling classified information and systems.
Additionally, all resources (e.g. maps, publication/instructions, photos) provided by the government to assist the contractor in the performance of their contract will be surrendered upon termination of employment or the end of the contract performance period.
g. Non-Disclosure Agreements: To safeguard information, the contractor shall enter into non-disclosure agreements with the responsible local security manager.
h. Privacy Act: Work on this project requires that personnel have access to Privacy Information. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.
i. Contractors Working in Controlled or Restricted Areas: The unit requesting contract support will provide escort (s) for contractors’ at all times when within a controlled area.
Contractors shall not escort other contractor employees within controlled or restricted areas. In addition, contractors shall fulfill, maintain, and comply with all security requirements IAW AFI 31-101, Integrated Defense, and command/local directives.
j. Access to Government Facilities with Controlled or Restricted Areas The contractor shall comply with security regulations imposed by the installation commander and/or the agency responsible for the project location. Due to specific mission requirements inherent in the nature of controlled or restricted areas on Scott AFB, the government may direct the contractor to leave the controlled or restricted areas at any given time.
k. Contractors Responsibilities while Working within Controlled or Restricted Areas:
Only contractor personnel with proper authority and qualifications shall enter a controlled or restricted area. Certain facilities require the issuance of an AF Form 1199CD. The government, at any time, may revoke the AF Form 1199CD. The procedures for the issuance of an AF Form 1199CD are contained in SABFI 31-1 Integrated Defense/Antiterrorism Plan. Contractors shall not escort other contractor employees within controlled or restricted areas. Upon completion of the contract, all 1199CDs will be turned into the sponsor and in turn delivered to 375th SFS/S5B, Line Badges. In addition, contractors shall fulfill, maintain, and comply with all security requirements contained in SABFI 31-1 Integrated Defense / Antiterrorism Plan.
l. Access to Government Facilities with Controlled or Restricted Areas for
Replacement Contractors: The unit requesting contract support will continuously escort replacement contractors, not initially possessing the proper clearances and requiring entry to controlled or restricted areas. Replacement contractors shall submit paperwork within
30 days of being assigned to obtain an approved security clearance or favorable review.
Replacement contractor personnel shall obtain a security clearance prior to working with, or having direct access to, classified material. In addition, replacement contractor personnel shall obtain a “Favorable review” prior to having access to a Controlled Area.
The above information shall be submitted to 375 AMW/IP.
