SECTION 01 12 00

JOINT BASE SAN ANTONIO (JBSA) SPECIAL REQUIREMENTS

PART 1 GENERAL

1.1 SUMMARY

This Section covers project requirements that are unique to Joint Base San Antonio following locations (1) Fort Sam Houston and Camp Bullis; (2) Lackland AFB; and (3) Randolph AFB.

These unique requirements relate to items such as: Digging permit process; Utility outages; Road closures; Post/Base access; Backflow prevention assembly documentation; Utilities;

Installation guidelines; and Customer Service Inspection certifications; etc.

1.2 REFERENCES

The publications listed below form a part of this specification to the extent referenced. The publications are referred in the text by basic designation only.

1.3 SUBMITTALS

1.2.1 In the SUBMITTALS requirements of this section include the following Specification:

Government approval is required for submittals with a "G" designation; submittals not having a

"G" designation are for information only. The following shall be submitted in accordance with

Section 01 33 00 SUBMITTAL PROCEDURES:

SD-13 Certificates

Customer Service Inspection Certification; G.

The Contractor shall supply a "Customer Service Inspection" certificate for the water supply in accordance with the Texas Commission on Environmental Quality (TCEQ) Regulations. The completed and signed certificate shall be submitted to the Contracting Officer for review and final approval. A blank certificate is located at the end of this section. See paragraph

CUSTOMER SERVICE INSPECTIONS for additional information.

Backflow Prevention Assembly Tests; G.

Certification of proper operation of backflow preventers shall be as accomplished in accordance

PART 2 PRODUCTS – NOT USED

PART 3 EXECUTION (Permit Requests, Procedures and Information)

3.1 JBSA-FORT SAM HOUSTON (FSH) AND CAMP BULLIS (CB)

3.1.1 Spoils Area

JBSA-Fort Sam Houston’s spoils area is full and is no longer available to contractors. All borrow or spoil operations shall be off of Government controlled property at the contractor's responsibility and expense.

3.1.2 Base Access and Travel

Access to Joint Base San Antonio, Fort Sam Houston Person (s) not in possession of a valid access credential must enter the installation through the following entry control points:

a. Walter Street

b. Harry Wurzbach East

c. SAMMC IH35

d. Jadwin (Commercial vehicles only)

e. Camp Bullis

Commercial vehicles such as delivery vehicles, shop trucks, public utility, etc., with on-post visitor passes shall enter the installation through the Jadwin Gate.

3.1.3 Prohibited Haul Routes

a. Through any housing area (skirting the area is acceptable)

b. Through the Community Center (bounded to the North by Schofield Road, to the East by

Schaffer/Patch, to the South by Wilson St, and to the West by Stanley Road.

c. Do not travel on Stanley Road

d. Travel only on defined roads through the AMEDD Training Campus.

3.1.4 Gate Restrictions

All commercial Class A/B type vehicles must enter through the following gates which have equipment available to screen/inspect Class A/B type vehicles:

a. The Commercial Gate at Jadwin Road (If this gate is closed, typically after 1800, vehicles may enter through the Walters gate).

b. For SAMMC, all commercial vehicles must enter the IH-35 Gate Commercial Gate.

c. Commercial vehicles entering the IH-35 gate should have a destination within the

SAMMC Campus.

d. Arrangements may be made to waive access protocol to allow Class A/B type vehicles through other gates for special circumstances on a case by case basis. Each case needs a minimum 24hour notification to the Physical Security Branch, and requires the

Contractor name, and number of waivers required.

3.1.5 Gate Alternatives

The use of any “Contractor Only” or “Supplemental” gates is solely at the discretion of and subject to the approval of the 502nd Air Base Wing and 502nd Security Forces Squadron.

a. Contractor Only Gates operated by construction project contractors solely for all types of contractor traffic moving across the installation boundary into the project construction site without access to other parts of the installation. Contractor bears cost of infrastructure and fencing to ensure project construction site is physically isolated from all remaining parts of JBSA-Fort Sam Houston.

b. Contractor Gates are internal to the contractor’s operation and, while subject to inspection and oversight by the Provost Marshal/Physical Security Office to insure that access from the project site to the rest of the installation are precluded, do not require any inspection other than that imposed by the contractor. Security within the contractor’s area of operation is the responsibility of the Contractor.

3.1.6 Supplemental Gates

Additional gates operated by the FSH installation gate guard contractor with funding for guard personnel and the “physical construction” (road work, shelter, and all utilities) provided by the construction project. Potential locations for Supplemental Gates might include, but not be limited to:

a. Old Jadwin Road Gate (adjacent to/North of the Commercial Gate at Jadwin Road)

b. Scott Road North Gate [Scott Road at Harry Wurzbach], Petroleum/Holbrook Road

c. Binz-Engleman.

Supplemental Lanes/Gates operate under the same basic criteria as the existing commercial gates because vehicles passing through these gates have open access to the installation.

However, use of SUPPLEMENTAL GATES should be limited to contractor traffic associated with the construction projects that are paying for the operation of the gates.

3.1.7 Locked FSH Gates

Gates can be opened for emergency or unusual situations by contacting the Security Force

Squadron, Building 2250.

3.1.8 Contractor Support

Support of any nature through the JBSA- FSH and CB will be coordinated through the Corps of Engineers Area Office and paid for by the Contractor. Support refers to any assistance by

JBSA FSH personnel or use of JBSA FSH assets.

3.1.9 Permanent Water Meter Installation Requirement

A main water meter shall be installed by the Contractor at each building or facility. Water meters will be equipped with electronic or Radio Frequency (RF) transmitters for remote monitoring as specifically required by the contract

3.1.9.1 Potable Water (FSH CES)

a. Meter will be provided and installed by the Contractor.

b. Billing agreements – will be the responsibility of the JBSA- FSH CES.

c. Time Frame – 14 Days (will run concurrent with digging permit).

d. Procedure – See CES Construction Manager for instructions, digging permit, locating, etc.

3.1.9.2 Recycle Water – San Antonio Water System (SAWS)

a. Meter will be provided by the SAWS, installation will vary according to project.

b. The Government has established billing agreements with SAWS for water use after completion of the project.

c. Time Frame – Follow SAWS Recycle Program for planning. After approved plan is accepted by SAWS and FSH, the Contractor shall coordinate with the CES PM for instructions, digging permit, locating, etc.

d. Procedure – If there is ANY CONNECTION to SAWS Recycle Water on site, all potable water service connections in the vicinity of the project construction limits must be equipped with a reduced-pressure-principle backflow prevention assembly.

e. JBSA-Fort Sam Houston has a base-wide Recycled Water Service Agreement with

SAWS.

f. New projects on JBSA-Fort Sam Houston there will be installation/relocation expenses required for each project at the Contractor’s expense. However, there will incur no application fees, impact fees or connection fees for any Recycled Water projects.

g. Drawings and Specifications shall be required on each new Recycled Water tap/project.

Drawings will show all potable water lines, facilities buildings and other improvements as well as all equipment requiring recycled water.

h. A permit will be required for each Plan Review process, no fee will be accessed for this permit process. SAW’s mains and Service Department must approve the permit prior to construction.

3.1.10 Permanent Electrical Service

3.1.10.1 General

a. Both temporary and permanent power for buildings will be provided by CPS Energy.

b. The same application can be used for both gas and electric service. The application can be completed by calling the CPS Energy New Construction/Commercial Application

Office at (210) 353-3333. See Appendix “E” for application form.

c. When application for service is submitted, Contractor shall request IDR meters for buildings larger than 29,000 square feet from CPS Energy. These meters will allow FSH personnel to monitor electrical load data using CPS Energy’s Load Tracker data system.

d. CPS will perform electrical field surveys at the Contractor’s expense

e. The solicitation drawings show a suggested route for distribution based on CPS Energy standard and a separate Bid Item is included in this Request for Proposal with CPS

Energy’s preliminary cost for this work.

f. There will be an additional charge for any upgrades to the primary distribution system

(both overhead and underground) and any changes to the original suggested route shown.

g. After CPS Energy receives the D/B Contractor’s final approved and stamped electrical site plan, electrical one-line diagram and a load analysis, CPS Energy will provide a price for the distribution of electrical utilities. The D/B Contractor will negotiate, contract and schedule the work directly with and pay CPS Energy to do the approved work.

**Additional costs for:

1) Design Options or/

2) Resultant defects due to contractor designs AND EXECUTION shall be allowed only to the extent that these actions decrease net design, construction, and other costs to the government.

h. A modification will be issued to the D/B Contractor if the actual CPS Energy cost differs from the CPS Energy budget included in the contract.

i. CPS Energy will build the exterior aerial power distribution system and riser pole after payment has been received from the D/B Contractor.

j. CPS Energy will remove any existing utilities prior to demolition at the D/B Contractor’s expense.

k. The D/B Contractor must contact CPS Energy fourteen (14) days prior to the excavation of property or demolition of the utilities.

l. For the underground primary, the D/B Contractor will build the underground ducts (a minimum of 2-4” and 1-2”), concrete slab for the pad mounted transformer, and manholes to latest edition of the CPS Energy standards.

m. The D/B Contractor shall install conduits inside the concrete transformer slab for the primary and the secondary cables.

n. The D/B Contractor shall install CPS Energy furnished ground rods in the transformer slab.

o. CPS Energy will provide and install the pad mounted transformer, pull in primary cables, connect to aerial primary at the D/B Contractor’s expense.

p. The D/B Contractor shall install meter rack, secondary duct, secondary cables, 2” conduit form meter to DDC panel in building and connect secondary cables to building.

q. CPS Energy will make primary and secondary connections to transformer terminals

(contractor to provide secondary lugs) and install metering equipment at the D/B

Contractor expense.

r. An additional 2” conduit will be required between pad-mounted transformers and meter enclosures when applicable.

3.1.10.2 Engineering

Texas profession engineering stamp will be required on the following information

a. Transformer Detail – show perimeter clearance and the location of the meter. Provide written dimensions with respect to some existing, permanent structure to define transformer location.

b. Electrical One-line Diagram - show the number and size of the secondary conduit and wires.

c. Electrical Layout – show the route of secondary wires to the main distribution switch.

d. Electrical load analysis - show connected load and demand power requirements.

e. Utility Site Plan – show the transformer, riser pole, manhole(s), and preferred layout of the primary duct line.

3.1.10.3 CPS Energy Rebate Program

a. Contractor shall coordinate, follow the application process steps, and submit the application for any CPS Energy Rebate Program requested in the contract. See Appendix

“E” for instructions and application form.

b. Contractor shall follow up and provide the required information to CPS Energy. The government will work with the Contractor to complete the process and obtain the rebate.

3.1.11 Permanent Natural Gas Service

3.1.11.1 General

a. Permanent and temporary natural gas for buildings will be provided by CPS Energy.

b. The solicitation drawings show a suggested route for distribution based on CPS Energy standards. Final location of natural gas meter to coordinated with CPS Energy. CPS

Energy will install natural gas service main to meter. A separate Bid Item is included in the contract with CPS Energy’s preliminary cost for Natural Gas work.

c. After CPS Energy receives the Design Build (D/B) Contractor’s final approved natural gas site plan and a load analysis, CPS Energy will provide a final price for the distribution of natural gas utilities. The D/B Contractor will negotiate, contract and schedule the work directly with and pay CPS Energy to do the approved work.

d. A modification will be issued to the D/B Contractor if the actual CPS Energy cost differs from the CPS Energy Natural Gas preliminary estimate included in the contract.

Additional costs for:

1) Design Options or/

2) Resultant defects due to contractor designs AND EXECUTION shall be allowed only to the extent that these actions decrease net design, construction, and other costs to the government.

e. CPS Energy will remove any existing utilities prior to demolition at the D/B Contractor’s expense. CPS Energy requires a minimum time notification of thirty (30) days to remove or relocate existing natural gas utilities. The D/B Contractor will be coordinate and pay for the removal or relocation of natural gas utilities.

f. CPS Energy (or the qualified contractor of its choice) shall select and install all gas mains, services up to and including the gas meter and regulator set at the D/B

Contractor’s expense. These facilities are owned and operated by CPS Energy. CPS

Energy will provide and connect its own natural gas meters.

g. The D/B Contractor shall provide and install all gas piping and related facilities between the CPS Gas Meter and the building.

h. The D/B Contractor shall be responsible to locate all underground utilities prior to CPS

Energy work.

i. The D/B Contractor must coordinate the location and sizes of all gas sleeves with CPS

Energy. Note that open trenching of new sidewalks or roads will not be allowed.

j. The D/B Contractor will need to fulfill all requirements of the Digging Permit for JBSA-

Fort Sam Houston and coordinate installation with the USACE PM and CES

Construction Manager.

3.1.11.2 CPS Requirements

The contractor to provide the following information to CPS Energy to obtain natural gas service:

a. File an application for service. The same application can be used for both gas and electric service. The application can be completed by calling the CPS Energy New

Construction/Commercial Application Office at 210-353-3333. See Appendix E for application form.

b. Supply a site drawing of the project with orientation of structures on the property, indicate approximate location gas line will enter structure and dimensions of related areas.

c. Supply the required load demand information in cubic feet per hour (CFH) or British

Thermal Units (BTU) and the required pressure at the meter in pounds per square inch

(psi).

3.1.12 Permanent Telecommunication

3.1.12.1 General

a. All equipment, supplies and materials provided shall be new and not refurbished. All materials for the projects shall be weather and corrosion-resistant and all materials and installation to be Seismic Zone 0 compliant. The contractor shall comply with all

BICSI/ANSI/TIA/EIA. The contractor shall submit the following certifications with technical proposal: Registered Communications Distribution Designer (RCDD) and

Network Transport Specialist (NTS).

b. The Contractor shall provide three sets of As-built drawings upon project completion

(CAD and hard copy format) to 502 CS Project Manager, 502 CES Project Manager, and

Army Corp of Engineer Project Manager. The size of the hard copy format shall be 24” x

36” (Arch D) in accordance with specification section 01 35 13.20 00 Special Project

Procedures – CAD. The contractor is required to provide the updated Man Hole

Distribution System (MHDS) information in appropriate *.pdf format and in Auto CAD

(.dwg file) format. The contractor shall also provide geo-spatial data to the 502 CS

Project Manager in accordance with specification section 01 35 13.30 Special Project

Procedures – GIS so that government personnel can update the base drawings in (CIPS

Visualization Component) CVC format of this document. If required, the contractor shall pump out and clean any communications Manholes (MH)/ Hand Holes (HH) within the scope of this project to the degree necessary to provide accurate information.

c. Each cable installation must be coordinated with the 502 Communication Squadron (CS) so that the impact on building users has been properly coordinated. All fiber runs shall use continuous Single Mode fibers, with no intermediate patches, cross connects, and a minimum utilization of splices. Any splices that must be used shall be fusion splices. No mechanical splices are to be used.

d. The fiber cable shall comply with industry standards with regard to manufacturers’ cable markings, armor jacket, rip cords, filling compound, fiber color coding, jacketing materials, etc. Installed fiber optic cables shall not contain factory fiber splices.

e. 3.12.1.5 The Contractor shall provide 50 foot FOC maintenance loop in every 3rd

MHs/HHs and in MHs/HHs directly outside the buildings. The contractor shall provide the updated cable layout information in *.pdf and appropriate Auto CAD (.dwg file) formats in accordance with specification section 01 35 13.20 00 Special Project

Procedures – CAD.

f. During the project, the Contractor is responsible for the configuration management, and as-built drawing documentation of the installed system. The Project Manager/CVC technician may provide a GeoBase portable recording unit (Trimble Geo XT, or equivalent) to the contractor, and explain its use, but in any case the contract is responsible for collecting and reporting the required data in accordance with specification sections 01 35 13.20 00 Special Project Procedures – CAD and 01 35 13.30 Special

Project Procedures – GIS. The contractor shall then proceed to the new installation and document the new work by loading it into the Geo-Base portable unit. The CVC technician will then upload the recorded date into CVC for configuration control. If the upload is not acceptable, the contractor will be instructed to return to the unacceptable areas and record the missing data again. When new cable is installed and properly labeled, the contractor shall document the work by taking digital pictures of each manhole/hand-hole (all 4 sides), the building penetration points, and the final terminations. Clearly label each picture in coordination with the 502CS/PM. All pictures and shape files (from the GeoBase survey) will be recorded on CD and turned over to the

502CS /PM by project completion and submitted as required by the appropriate specification section. The contractor is responsible for repairing any damages to the infrastructure. The contractor is also responsible for coordinating all actions (bldg & main telecommunication room access) with the building manager through the 502nd

Construction Manager and Quality Assurance Representative.

g. Due to operational considerations, fiber/copper cable installation shall be coordinated with the 502 Communication Squadron (CS) Project Manager (PM) to ensure there are limited interruptions due to exercises and other base activities. Contractor access to communications nodes, hand hole, manholes, and pedestals shall be coordinated with the

502 CS Project Manager.

h. If portions of existing Hand Hole Distribution System (HHDS)/MHDS are not useable

(other than simple clogging due to removable mud, dirt, or debris), the contractor is responsible for restoring the HHDS/MHDS to use. The contractor is encouraged to propose minor changes to the recommended route.

i. The contractor shall label all telecommunications infrastructure and equipment components. All installed Fiber Optic Cable (FOC) and copper cable shall be numbered following a site cable plan and documented in as-built drawings and documentation.

Identification marking and systems shall be uniform, permanent and readable and consistent with established base labeling schemes IAW ANSI/TIA/EIA-606 standards and direction from 502CS. New penetration points in buildings shall be permanently labeled indicating the HH/building the new duct connects to. All tags shall be

10 Aug 2015 JBSA Special Requirements Page 11 of 18 permanently labeled, easily visible and corrosion resistant. Install cable tags in all

MHs/HHs and Building Entrance Terminal (BET) locations. When cables pass through a

MH/handhole put a tag on the cable, approximately 2 feet from each duct entrance.

Information on the cable tag shall identify cable by size, type, cable number and count.

j. Outside plant (OSP) multipair cable shall be REA/RDUP (Rural Electrification

Administration/Rural Development Utilities Program) Specification PE-39. No conductor size less than 24 AWG shall be proposed. Gopher (armor) protection is not required for cable to be installed in duct. The cable shall comply with telecommunications industry standards with regard to manufacturers’ cable markings, jacket, rip cords, filling compound, color coding, jacketing materials, etc. All cable shall be rated for the environment in which it is installed and shall be in compliance with the current issue of the National Electrical Safety Code (NESC) and the National Electrical Code (NEC).

k. Splice closures designed specifically for buried applications shall not be used in underground applications. Only splice closures intended for underground applications shall be used in the underground system. The splice closure shall be suitable for pressure testing and shall have a rigid end plate to provide a seal against the cable sheath for applications at locations where the possibility of immersion exists. The underground closure shall be suitable for enclosing straight, butt, and branch splices in a protective housing. It shall protect the splice and maintain cable shield electrical continuity in a manhole environment. 502 CS practices require that the closure shall be Preformed Line

Products (PLP) stainless steel with filling flange. Note: It is not anticipated that any buried splices will be required, but if any are, they shall be sealed with Preformed Line

Products stainless steel splice case with filling flange.

l. All conduits, inner-duct, sleeves, and penetrations shall be fire stopped in accordance with ANSI/TIA/EIA-569-A Annex A and all applicable local codes. Inner-duct shall be fire stopped with approved soft materials (mineral wool or moldable putty) at both ends and at intermediate connection points unless these points terminated in a 2-hour fire-rated enclosure. Conduits and sleeves that contain fiber or unshielded twisted pair (UTP) shall be fire stopped using the appropriate method for the individual location.

m. Grounding and bonding as defined by National Electric Code, local building codes for electrical materials, TIA/EIA-607, MIL-STD-188-124B and construction methods shall be strictly adhered to for new networking systems installed.

n. All residues from the project shall be disposed of off base and IAW Federal, State, local, and FSH environmental laws and regulations.

o. The Contractor shall use Global Positioning System (GPS) equipment and technology to locate and collect geospatial and attribute data on outside plant infrastructure features and communications pathways that are installed or modified as a result of the Contractor’s

Outside plant features to be located include MHs, vaults, HHs, pedestals, underground cables, direct buried splices, terminals etc. The communications pathways include duct routes, direct-buried cable, and communication pathway entry points on building exterior walls that lead to interior building entry terminals, etc. The

Contractor shall augment the GPS survey by using conventional land surveying equipment and electronic cable locating (underground utility toning) equipment when necessary to accurately locate features. The Contractor shall convert the GPS data to an

ESRI Shape file set suitable for the Government to load the file and transcribe information from the Shape file into the CVC database. All submitted information shall meet the requirements of specification sections 01 35 13.20 00 Special Project

Procedures – CAD and 01 35 13.30 Special Project Procedures – GIS.

p. Each outside plant infrastructure feature and communications pathway requiring geospatial coordinates shall be located to within one meter of its true ground position, in the horizontal plane as defined in Federal Geographic Data Committee; FGDC-STD-

007.3-1998 and as verified by accuracy worksheets. Geospatial coordinates for the location of MHs and cable vaults shall be recorded for the center of the manhole lid.

Geospatial coordinates for the location of HHs, pull boxes, pedestals, and marked direct buried splices shall be recorded for the top center of the feature. Geospatial coordinate data for the location of communication pathways lines shall be recorded at a minimum every 50 feet along the pathway, and at each turn or bend in a cable installation pathway a sufficient number of data points must also be collected and recorded so that the coordinates for any point along the pathway will fall within the allowable accuracy.

q. The Contractor shall use a data dictionary in the GPS data loggers by using the entity naming convention, attribute fields and domain values as specified 502nd

Communication Squadron. The GPS collected outside plant infrastructure features and communications pathways attribute, metadata in accordance with FGDC-STD-001-1998, and the GPS location and feature attribute information shall be converted to, stored, and submitted in Microsoft Excel, XML and Environmental Systems Research Institute

(ESRI) Shape File formats as applicable. The Contactor shall process the Shape files with a topology build and clean routine software application. The Shape file shall be compatible with the FSH GeoBase Common Installation Pictures in accordance with specification section 01 35 13.30 Special Project Procedures – GIS.

r. The Contractor shall provide geospatial data deliverables for new excavations and new outside plant infrastructure features. Geospatial deliverables include Shape files, metadata and accuracy worksheets.

s. The contractor shall provide photograph deliverables for new excavations and objects.

Photographs shall also be provided for all existing infrastructure that was modified and must depict all directions of the object with the file accurately labeled. All digital photographs shall be submitted.

t. At the completion of the project, the contractor shall provide a summary report in MS

Word format describing their installation in detail, identifying any problem areas, and providing any recommendations for future installations.

u. The Contractor shall perform testing and inspections of all systems solutions to ensure the technical adequacy and accuracy of all work, including reports and other documents required in support of that work. The contractor shall conduct on-site testing IAW

Original Equipment Manufacturers (OEM)’s installation manuals, practices and the appropriate vendor’s test procedures. The contractor shall participate with the

Government in testing the complete communications system.

v. The Contractor shall furnish all test equipment and personnel required to conduct all required testing. The contractor shall document all test results in a “Test Report(s)” and submit to the Government. During the testing phase, the Government reserves the right to perform any of the contractor performed inspections and tests to assure solutions conform to prescribed requirements. The contractor shall provide on-site support during the acceptance testing. The Test Report(s) shall be provided to the Government no later than

5 days after test(s) have been completed.

w. Acceptance tests for both fiber optic and copper cable facilities installed will be performed in accordance with I3A Standard, Para. 3.19. Additionally, all single mode

(SM) FOC installed shall be tested in accordance with ANSI/EIA/TIA 0-526-7(OFSTP-

7), Para. 5.1 Method A.1. Contractor shall calculate passive Cable System Attenuation loss on the link(s) being installed. Calculated loss will total the sum of fiber cable attenuation coefficient (db/km) X Length (km), connector attenuation (db), and splice attenuation (db). This calculated loss shall accompany test readings and will be the maximum acceptable reading for the Government. Attenuation coefficients for fiber cable, connectors and splices are available from the ANSI/EIA/TIA 568 Standard. All test results must be reviewed and approved by the assigned 502CS/SCXP (PM) and 502

CS/SCQ (QAE) prior to completion of this project. Any identified test results not meeting industry standards must be corrected, retested, and resubmitted for final review prior to payment by the US Government.

x. Upon completion of installation and in-progress tests, pre-cutover tests shall be accomplished to ensure the entire system is properly configured, the requirements of the task order have been met and the system is ready to cutover.

y. The Contractor shall coordinate and contact the 502nd Communication Squadron for approval of the exact locations and orientation of conduit penetrations within the existing manholes prior to starting work.

z. Provide a separate data fiber/copper connection (including cabling and conduit) for the

HVAC to the UMCS that will be connected to the UMCS system.

3.1.12.2 Commercial Cable Transmission Media

a. JBSA-Fort Sam Houston has a privatized, contractor run commercial cable TV network.

Cable is RG6. Terminate at jack on standard TV outlet. Place termination equipment in rack or on back board.

b. Contractor will provide secure space or commercial closet for all commercial cable TV equipment. No commercial cable TV or customer related equipment will be housed in the government main telecommunication rooms (MTR)/ telecommunication room (TR).

c. Provide minimum of 2 - 2” conduits, underground through floor slab, for commercial cable TV/telephone. It shall be installed from commercial/utility closet and extend five

(5) feet beyond the exterior face of the building (approved by the COR). All ducts shall be capped on the exterior and fire stopped on the interior of the building. Exterior building placement (stub out) shall be marked with a metallic end location.

3.1.13 Contractor Staging Areas (Temporary Construction Facilities and Controls)

The contractor’s staging area will be within the contract grading limits. Proposed staging areas and concrete plant sites outside the grading limits will be approved prior to indicating the location in the bid documents. If the contractor requires a staging area outside the grading limits or in conjunction with renovation of existing facilities a request showing the limit of the staging area must be submitted for approval prior to use of such area. All staging and storage areas shall be fenced and screened. All staging areas near residential areas shall be fenced and locked during non work hours.

3.1.14 Recycle Materials (Construction Waste Management)

There is no current recycle program on JBSA-Fort Sam Houston. However, the Contractor shall recycle any material feasible. Documentation of material recycled must be reported to the Government including the type and approximate amount salvaged. The Contractor shall be responsible for removing all unused material and refuse from JBSA-Fort Sam Houston and disposed of appropriately.

3.1.15 Disposal Construction Waste (Construction Waste Management)

Unless specifically shown or required otherwise, all unused material, refuse, concrete and asphalt rubble resulting from demolition or site improvements shall be removed from JBSA-

Fort Sam Houston at the expense of the Contractor and disposed of appropriately.

3.1.16 Phasing Plan (Alterations to Existing Facilities)

Where demolition of multiple building construction requires phasing, the phasing shall be established prior to and published for advance final review.

3.1.17 Customer Service Inspections (Water Connections)

a. The Texas Commission on Environmental Quality (TCEQ) Rules and Regulations for

Public Water Systems requires a "Customer Service Inspection Certification (example, Appendix F) be completed prior to providing continuous water service to new construction, on the existing system when the water purveyor has reason to believe cross-connections or other unacceptable plumbing practices exist, or after improvements, corrections, or additions to plumbing facilities." This rule was effective 1 Jan 96.

a. Personnel qualified to make the certifications are plumbing inspectors licensed by the

Texas State Board of Plumbing Examiners, certified waterworks operators, and licensed plumbers (for single family residential services).

b. The Customer Service Inspection records must be retained 10 years and be available for commission review. Please forward all completed inspection forms for work completed after 1 Aug 98 to CES Construction Manager.

c. Backflow prevention tests are monitored by the CES Sanitation Shop. The Contractor shall test backflow prevention assemblies as required by 31 TAC 290.44(h). At the completion of inspection, the Contractor must submit a Test Report in compliance with

TCEQ: 30 Texas Administrative Code (TAC) Chapter 290.47(f), see Appendix G. The

Contractor shall verify and assure the products specified as capable of being field tested, i.e., have test ports, cocks, etc., as needed. Any use of recycled water shall be done in compliance of SAWS and TCEQ.

d. Certification Requirements The Rules and Regulations for Public Water Systems (TCEQ publication number RG-195) require a customer service inspection certification before providing continuous water service for the following conditions: New construction involving plumbing or mechanical systems; or Material improvement, correction, or addition to plumbing or mechanical systems.

3.1.17.1 Inspection

The Customer Service Inspection certifies that for work done under this contract:

a. No direct connection between the public drinking water supply and a potential source of contamination exists;

b. No cross-connection between the public drinking water supply and a private water system exists;

c. No connection exists which would allow the return of water used for condensing, cooling industrial processes back to the public water supply;

d. No pipe or pipe fitting which contains more than 8.0 percent lead exists in private water distribution facilities installed on or after July 1, 1988; or

e. No solder or flux which contains more than 0.2 percent lead exists in private water distribution facilities installed on or after July 1, 1988.

3.1.17.2 Inspection Personnel

Customer Service Inspections must be performed by personnel meeting the requirements described in the Rules and Regulations for Public Water Systems. Examples are individuals licensed by the Plumbing Examiners Board (Licensed Plumbers or Plumbing Inspectors) or

Certified Waterworks Operators and members of other water-related professional groups who hold an endorsement granted by the commission or its designated agent (have been to the 10 hour training approved by TCEQ).

3.1.17.3 Inspection Certification Form

Original copies of the Customer Service Inspection Certification shall be provided to the

Contracting Officers Representative prior to final inspection and acceptance. Certification forms will be maintained by the JBSA-Fort Sam Houston CES Water and Wastewater Utility representative. A sample form is provided in Appendix F

3.1.18 Hazardous Material (Alterations to Existing Facilities)

Where alterations or rehab work requires work done in or to existing buildings, the

Contractor shall ensure that a proper survey of all potentially hazardous materials are done and proper specifications for removal and disposal are included in the project. JBSA-Fort

Sam Houston CES requires that materials generated by demolition be disposed of by the

Contractor when allowed by disposal regulations. Provide lists of hazardous material to be disposed of on drawings or in specs.

3.1.18.1 Asbestos and Lead Notification Requirements

Asbestos (Licenses, Permits and Notification):

a. The contractor shall be responsible for completing the Department of State Health

Services (DSHS) 10-day asbestos notification. Once notification package is complete, it must be conveyed (original signature only) to (DSHS). Note: 10 working days.

b. Once the DSHS notification has been completed and returned. The contractor must forward the notification to JBSA-Fort Sam Houston's (FSH) Construction Manager

(CM). The (FSH) (CM) will then review the notification package for completeness and accuracy. The following minimum information must be included on Transmittal:

Building # and address (construction site)

Asbestos abatement contractor's contact information.

c. The (FSH) (CM) will forward the notification package to the 502d Civil Engineer

Squadron, Environmental Office at the following address:

d. The notification package will be logged-in and routed to Mr. Leandro Quiroga (Mike)

(Asbestos and Lead Manager). If the asbestos notification package is found to be in error, the (FSH) (CES) (CM) will be notified that the package must be corrected and resubmitted. If the package is found to be complete and accurate, it will be signed off by the Asbestos and Lead Manager. The (FSH) (CES) (CM) will then be notified that the package is ready for pickup.

e. If start and completion dates are not met, the contractor will submit an amendment to the original state notification. The (CES/EO) will sign and return to the contractor.

f. f. The (FSH) (CES) (CM) will then forward documentation to the contractor. The contractor must deliver the signed Original asbestos notification package to DSHS in a manner, and within the time frame required by IAW TEXAS LAW.

3.1.18.2 Asbestos (Disposal Documentation):

a. The contractor shall prepare the manifest for disposal.

b. The contractor shall prepare all waste profile documentation for the acceptance of waste into the disposal facility.

c. The profile sheet must be signed by the JBSA-Fort Sam Houston (CES) Environmental

Office (EO).

d. The contractor shall use the FSH's Environmental Protection Agency (EPA) identification and state registration numbers.

e. The manifest shall be returned to the (FSH) (CES) Environmental personnel as the generator.

f. The manifest shall be returned to the (FSH) (CES) Environmental Office no later than 30-days after disposal materials have been shipped.

g. The original manifest must be mailed to:

1.4 SPECIAL SCHEDULING REQUIREMENTS

a. Contractor must be ready for operation as approved by Contracting Officer before work is started which would interfere with normal operation.

b. Have materials, equipment, and personnel required to perform the work at the site prior to the commencement of the work.

c. All medical and mission critical areas will remain in operation during the entire construction period. The Contractor must conduct his operations so as to cause the least possible interference with normal operations of the activity.

d. Permission to interrupt any Activity roads, railroads, and/or utility service must be requested in writing a minimum of 30 calendar days prior to the desired date of interruption.
