STATEMENT OF WORK

FOR

Maintain Shop and Youth Center Floors B411/304/306

EEPZ 98-2021X

1 June 2018

I. PROJECT INFORMATION GENERAL SCOPE

A. The Contractor shall provide all plant, labor, materials, equipment, transportation, and tools required to execute preparation and refinishing of the concrete floors and repaint existing floor lines in the specified locations at Buildings 304, 306, and 411 at Columbus Air Force Base.

Perform all work according to this Statement of Work (SOW), the Concept Design Drawings, and all related AF requirements and other codes and references.

II. DESIGN

A. The attached Concept Design Drawings are not more than 15% complete.

B. Verify constructability and suitability of Concept Design and SOW. Contractor shall field-verify all information related to existing conditions including: dimensions, calculations and code requirements for all elements of work. Contractor is responsible for all remaining design to provide Construction Documents that are in accordance with both statement and intent of SOW and Concept Design Drawings and are acceptable to the Government. Contractor shall provide 100% Design Submittals for review, comment and approval by CAFB. As determined by CAFB and usually within two weeks after each design submittal, the contractor shall engage in a design conference at 14CES.

III. OUTLINE of WORK

Provide all design and construction necessary to…

A. Interior

1. Building 304:

a. Diamond grind approximately 10,135 square feet of concrete flooring at the specified locations in the design drawing. All dust, dirt, or other debris that is released during this process must be contained and disposed of in accordance with all federal, state, local, and United States Air Force regulations, rules, and statutes. Due to the geometry of the building, hand grinding may be required around the walls and in-place equipment. All mobile equipment shall be moved by the government prior to the start of work.

b. Saw cut, clean out, and fill all concrete joints with polyuria joint materials.

c. Fill in any major holes and eroded concrete from the concrete flooring.

d. Ensure that the floor surface is prepared in accordance with manufacturers specifications for any product being used to re-finish the floor.

e. Apply a primer coat of Tennant ECO-GPE general purpose epoxy, or approved equal, at 400 square feet per gallon. If a material is selected other than Tennant ECO-GPE general purpose epoxy, the contractor must provide validation of equality to the Tennant ECO-GPE general purpose epoxy.

f. Apply a build coat of Tennant ECO-GPE general purpose epoxy, or approved equal, at 133 square feet per gallon, in color. If a material is selected other than Tennant ECO-GPE general purpose epoxy, the contractor must provide validation of equality to the Tennant ECO-GPE general purpose epoxy. The color shall be selected by the base architect.

g. Apply a build coat of Tennant ECO-HTS high traffic system urethane, or approved equal, at

500 square feet per gallon, in color. If a material is selected other than Tennant ECO-HTS high traffic system urethane, the contractor must provide validation of equality to the Tennant ECO-HTS high traffic system urethane. The color shall be selected by the base architect.

h. Repaint all existing lines in the original locations. There is approximately 176 linear feet of 6” wide yellow lines, 24 linear feet of 3” wide yellow lines, 191 linear feet of 4” wide yellow lines, 33 linear feet of 8” yellow lines, and 30 linear feet of 6” wide red lines. The color of the repainted lines shall match existing, and shall be validated by the base architect.

2. Building 306:

i. Diamond grind approximately 1,600 square feet of concrete flooring at the specified locations in the design drawing. All dust, dirt, or other debris that is released during this process must be contained and disposed of in accordance with all federal, state, local, and United States Air Force regulations, rules, and statutes. Due to the geometry of the building, hand grinding may be required around the walls and in-place equipment. All mobile equipment shall be moved by the government prior to the start of work.

j. Saw cut, clean out, and fill all concrete joints with polyuria joint materials.

k. Fill in any major holes and eroded concrete from the concrete flooring.

3. Building 411:

q. Diamond grind approximately 3,310 square feet of concrete flooring at the specified locations in the design drawing. All dust, dirt, or other debris that is released during this process must be contained and disposed of in accordance with all federal, state, local, and United States Air Force regulations, rules, and statutes. Due to the geometry of the building, hand grinding may be required around the walls and in-place equipment. All mobile equipment shall be moved by the government prior to the start of work.

r. Saw cut, clean out, and fill all concrete joints with polyuria joint materials.

s. Fill in any major holes and eroded concrete from the concrete flooring.

B. General Design & Construction Requirements:

1. Basic Items of work are as follows:

IV. Within the area of work are multiple long drains. The contractor shall protect these drains from dirt, dust, and/or debris caused by the work of the contractor.

V. The contractor shall remove all debris and dust resulting from the entire project.

VI. The contractor shall be responsible for validation of all measurements VII. The Contractor shall maintain Housekeeping practices, to include: proper storage, use, and disposal of materials (i.e., petroleum products, paints/solvents, acids, and detergents); sanitary waste management (i.e., portable toilets); and trash/debris containment (i.e., prevent entry into nearby waterways and storm drains).

VIII. Protect items not in contract from damage due to contractors work. All damage done by the contractor will be repaired by the contractor at their own cost without reimbursement.

IX. At the conclusion of the project the Contractor will be responsible to complied and furnish the CO, DD Form 1354, “TRANSFER AND ACCEPTANCE OF MILITARY REAL PROPERTY”, indicating all costs and quantity data of materials and systems furnished and installed. A draft of DD 1354 will be furnished to the Contractor who will then complete the square-foot and/or cost information for each item. The completed DD 1354 will be turned into the CVO on the date of final inspection. DD Form 1354 itself will be provided by the CO.

X. SCHEDULING

Construction work must not disturb the ongoing operations of the existing building. Coordinate interior work in the existing building with CEN. The government does anticipate significant amounts of work required during other than normal business hours to meet project schedule. The government may, at its sole discretion and on a case-by-case basis, require Contractor to perform certain tasks during nights or weekends. These will typically be limited in nature, and will normally include work that impacts base mission, populace, and traffic in a negative way. Examples of work that may fall into this category include but are not limited to: utility outages, short-term closure of major roads or gates, or hazardous material abatement in occupied facilities.

The Construction shall be phased as shown in the design drawings. When one phase is complete, the contractor shall return that side to the government for use, after a final inspection, and shall begin work on the following phases. The contractor shall submit an estimated schedule showing the breakdown of days required to complete each phase during the submittal process, prior to the start of construction to allow the Government to move the in-place equipment out of the areas of work.

XI. GOVERNMENT FURNISHED EQUIPMENT OR MATERIALS

The Government will not furnish any equipment or materials.

XII. OTHER

The Contractor shall immediately notify the Contracting Officer and secure any documents found with classified, “For Official Use Only” (FOUO) or personal information. The Contracting officer will direct disposition of these documents.

A Form 66 (attached) submittal is required.

Provide a one-year warranty on materials and labor for this project to assure good quality workmanship and materials and to provide necessary maintenance and correction of deficiencies during the year following contract competition. The contractor will issue to the Government any commercial warranties on materials or equipment included in the construction that exceeds this one-year, all-inclusive warranty.

Provide safe work area including adequate lighting and required noise protection while personnel are engaged in work. No hazardous materials are anticipated to be associated with this project. Contractor shall install proper construction markings and tape during work in progress. Contractor shall maintain a hazard free work area during operations. Upon receipt of Notice to Proceed, contractor shall notify the Contracting Office prior to starting any work.

Contractor shall provide Contracting Officer prior notice to starting any work that may affect any building function.
