STATEMENT OF WORK
Dormitory Twin Mattresses
Delivery, Installation, and Disposal for Technical Training Facility at
JBSA Lackland Student Dormitory Building 10504
PROJECT DESCRIPTION: The Contractor shall provide 600 dormitory twin mattresses for the Technical Training Dormitory at Joint Base San Antonio (JBSA), Lackland dormitory living facility throughout building 10504. The Contractor shall provide all transportation, personnel, labor, supervision, tools and incidentals for the delivery and installation of all 600 twin mattresses.
The Contractor shall remove and dispose of all the existing mattresses currently stored within the facility in accordance with (IAW) industry standards and local, state, and federal regulations. The contractor shall remove all existing mattresses off base at the contractor owns expense.
The installation and removal process shall be done IAW the project phases listed below with minimum disturbance to the training students residing throughout the facility. The Contractor shall do a one (1) for one (1) swap, replacing the old mattress with the new mattress as instructed by the onsite listed Program Managers (PM). The Contractor shall consult with the PM for all project concerns.
SITE LOCATION: The facility site shall be at building 10504 with a physical address of 1435 Metzger Dr. JBSA Lackland, TX 78236.
SITE DESCRIPTION: JBSA Lackland Building 10504 is a four (4) story dormitory building with 300 dorm rooms, all located with access from within the facility and each consists of two
(2) students per room. There are a total of 4 interior stairwells and an elevator, the contractor will not have access to elevator and shall use stairwells during the duration of this project.
Building 10504 be occupied with some students for the duration of this project. All personal items shall be secured and locked away.
SITE DELIVERY & INSTALLATION:
The contractor shall deliver, install, remove, and dispose of the mattresses IAW this Statement of Work throughout JBSA Lackland building 10504 with physical address located at 1435 Metzger Dr, JBSA Lackland, Texas 78236:

PROJECT PHASES: The contractor shall work closely with the PM to ensure the installation and removal process is completed in a satisfactory manner with limited disturbance to the students. Of the four (4) floors throughout the facility, the contractor shall work to clear one floor at a time and ensure the removal and installation of mattresses items are complete per room, one (1) for one (1) mattress swap, as the students are physically away from the facility attending required training The contractor shall begin in order of first, second, third, and fourth floor and clearly notify the onsite PM accordingly as they complete each floor.

• Warranty shall be industry standard not to be less than 1 year
PROJECT MANAGEMENT: The contractor shall identify and provide a single Point of Contact (POC) to provide overall management, supervision, and detailed communications throughout the term of this project. The contract POC will serve as a representative for the PM customer to ensure the delivery, installation, and disposal of mattresses is successful and
- 3 -completely in accordance with this statement of work. All pertinent communications changes should be confirmed in writing.
The awarded contractor shall provide the Government/Program Manager (PM) a 7–14-day notification prior to delivery, a 72-hour notification prior to truck arrival. The contractor shall also provide an email notification as to the date and time the trucks will be arriving to the Government Representative/PM installation facility, building 10504.
GOVERNMENT REPRESENTATIVE / PROGRAM MANAGER: The primary
Government Representative/Facility/PM shall be, Mr. Cleophus Sullivan/Jesse Vick and shall be contacted only during regular office hours at office.
ADDITIONAL PROJECT INFORMATION:
- All materials and accessories shall be installed in accordance with industry standards and manufacturer’s recommendations. Space can be made available to allow for staging or storage on the site’s parking lot, but the contractor must coordinate with the PM upon award. The contractor will also coordinate and confirm all deliveries and installation times with the PM no later than 72 hours prior to delivery.
- As of now, there are no conflicting projects taking place, however, the Contractor may need to made adjustments for delivery, installation, and disposal of the items required by this SOW should a conflict occur.
- Contractor shall provide their own dumpsters for contractor generated refuse throughout this requirement. All contractor dumpsters shall be in the area outside of building perimeter and not placed onto any grass surface and must be emptied as so as it is full and/or by the end of the business day. The government will not provide any dumpsters and will not provide any equipment for implementation and completion of this requirement.
ACCEPTANCE/INSPECTION: The contractor shall manage ordering, shipment, delivery and installation of all new mattresses until final acceptance by the designated Government PMs. The Contractor and PM(s) shall verify and inspect all items to ensure that no damage has occurred to the mattresses during delivery and installation.
QUALITY ASSURANCE: The Government PM shall inspect all work performed by the contractor. The inspection shall be completed by the Government representative/PMs and reported to 502 CONS representative as soon as possible.
The contractor shall deliver all Items specified, deliverables shall be new and not used or factory refurbished. The Government may require replacement of nonconforming supplies or performance of non-conforming services at no increase in the contract price. Any disputes in this area must be brought to the attention of the PM and the Contracting Officer immediately.

COMPLETION OF WORK: The contractor shall ensure that work for this project is performed in accordance with the criteria herein, and that all relevant materials, finishes, equipment, and systems shall be fully operational at the completion of work for this project.
In order for the project to be considered completed, work performance shall be deemed satisfactory and acceptable to the government. Work performance shall be deemed satisfactory and acceptable by the government when the Mr. Vick and/or Mr. Ingram declares the project satisfactory and complete. Thereafter, the contractor shall submit voucher for payment via the Wide Area Workflow processes.
HOURS OF OPERATION AND FURNISHINGS MOVEMENTS: The awarded contractor shall perform all work during the hours of 7:30 AM to 3:30 PM, Monday through Friday. Any related work required outside of normal business hours shall be requested from and approved by the Contracting Officer. The awarded contractor is not required to perform work during federal holidays such as the following, i.e., (New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, etc.).
PROTECTION OF BUILDING PROPERTY: The contractor shall provide all material to protect all floors, carpeted areas, walls, carpeting from damage and soil during the moving and installation. The protective materials shall not be bolted, glued, or stapled to floors, carpeted areas, walls, ceilings, or doors but instead shall either be free standing or braced so as not to cause any damage.
Proper measures (protective runners, pads, etc.) shall be provided and utilized by the contractor to protect the building (stairs, stairwells, walls, floors, etc.). The contractor shall establish and maintain this protection and maintain it throughout the full term of the purchase order. Any damage caused by the contractor shall be the responsibility of the contractor to repair or replace immediately or as agreed upon by the PM and/or Contracting Office.
After the installation of each phase, the contractor and government representative/PM shall jointly conduct a walk-through and annotate any new damage at the new location. Any damage caused by the contractor will be the responsibility of the contractor to repair or replace at no additional cost to the Government.
WORK SITE: Throughout the mattress’s movement process, the awarded contractor shall collect all trash, debris, refuse, garbage, etc., which is generated, remove, and properly dispose of all applicable materials from the work site. Trash and debris collection shall be coordinated by the contractor and disposed of off base. The contractor shall ensure required equipment is available for removal of debris, refuse, and garbage before and during the delivery of the dorm mattresses. The contractor shall take precautions to prevent damage to government property.

The contractor shall repair or replace damaged items resulting from contractor performance of this requirement at no additional cost to the government. It is incumbent for the contractor to identify any existing damage to the building by means of a Pre-Installation and Delivery Walk-Thru and notify the Government Representative/PM on site of any damages that were pre-existing prior to installation as well as any damage resulting in contractor performance. The PM on site shall also do a Pre-Installation and Delivery Walk-Thru and document any existing damage to the building as well as any damage resulting in contractor performance.
Additionally, any packaging trash shall be properly disposed of outside the installation in authorized disposal containers
WARRANTIES: Contractor shall provide all commercial warranties for a period of not less than one-year from the date of installation/acceptance/delivery for all issued mattresses items.
ACCESSORIES AND INCIDENTALS: Contractor shall provide all incidentals necessary to perform as required in accordance with this SOW. Cost for these items shall be included as part and parcel of the finished installation.
INSTALLATION ACCESS: The awarded contractor and all employees shall meet minimum security requirements to gain access on to Joint Base San Antonio (JBSA) Lackland, Texas. A ten (10) day advance notice is needed for a base access pass, and any person with unfavorable background history may not be able to enter the military installation. A base access pass for an employee without a U.S. Driver’s License or US State issue ID card may take longer.
The contractor shall obtain all required installation passes for contractor personnel by providing the employees full name, date of birth, Driver’s License/ID information and Country born into the Government Representatives/PMs identified within this document.
Contractor personnel shall be able to produce personal identification when required by current force protection conditions or special instructions.
The contractor personnel shall wear an identifiable outer garment (company shirt) or badge which clearly identifies a contract employee. The badge shall contain a name of employee and contractor's name and/or special company identifier when a/o if confronted.
CLEAN-UP: All debris and rubbish resulting from this requirement shall be disposed of by the contractor off Government property. The Government will designate a location for a contractor provided dumpster, if necessary. Disposal shall be compliance with all applicable Federal, State, and local hauling and disposal regulations. Site clean-up shall occur daily.

Contractor shall ensure a thorough cleaned up work site at the end of each working day prior to departing the installation.
