1. Is there a size limit on the trucks to be transporting equipment and waste across the Base?
a. No.
2. Will alternate gates be available if any events held by the city block or slow the main route?
a. Contractor trucks and equipment shall enter base through the contractor gate. If route is blocked, alternative gate entry will be arranged by the base.
3. Will there be any weight restrictions on trucks traveling on the Base?
a. Standard urban weight restrictions.
4. Is rail a possibility for transportation of waste?
a. No.
5. Is a formal dust control plan required?
a. Yes. Contractor may use nearby hydrant for dust control but must use backflow preventer and completely shut off valve with each use.
6. Will the contractor or Vectrus/CEV be responsible to write the dust control plan?
a. Contractor.
7. Are there any underground tanks within 200 feet of the demolition site perimeter or within the perimeter?
a. There is an underground collection tank underneath mechanical room. This tank may contain biohazardous materials and materials such as mercury and amalgam. Contractor must flush tank and lines. CE Environmental will test samples after flushing.
8. What volume and pressure of water is available for watering the sod?
a. Keesler water pressure is typically 40 - 70 psi.
9. Is there currently a water connection in place that will remain and be useable for the sole purpose of watering the sod?
a. Contractor may use nearby hydrant.
10. Will the 60-day watering period be extended beyond the performance period?
a. Yes.
11. Who will be responsible to mow the grass during the 60-day watering period?
a. Contractor.
12. Was this building or the site used as anything other than a dental office in the past? If so, what?
a. No.
13. Were there ever any recordable or known spills of any kind on any part of this site in the past?
a. There are no records available.
14. Were there any sewer leaks known to be under the building in the past?
a. There was a sewer leak 5 years ago and main sewer line was replaced underneath building.
Tertiary lines were not replaced.
15. Were hazardous or radioactive materials of any kind ever stored on this site?
a. Yes.
16. Has plumbing/sewer piping been included in any environmental testing?
a. No.
17. Has plumbing/sewer been tested for mercury and other heavy metals?
a. No.

a. There are no original architectural drawings. Attached to this response is a copy of existing power plan.
21. Who is responsible for all underground locates of the entire demolition site and surrounding area?
a. Vectrus and City Light and Power (CRP) will locate utilities before demolition.
22. Is depth of the existing utilities available?
a. No.
23. Has a ground penetrating radar analysis been performed, including under the building, on the entire site? If so, are the results available?
a. No ground penetrating radar has been performed at the site.
24. Will plumbing pipes require sealing of each section as they are removed?
a. Yes.
25. Will expanding foam installed in the pipes before demolition act as an acceptable seal?
a. Yes.
26. Are complete mechanical and electrical drawings available for the entire building?
a. There are no mechanical drawings available. Original power plan is attached.
27. Who is ultimately responsible to assure ALL alarms, monitoring systems, surveillance equipment, fiber optic cables, power systems, fuel systems, control systems and communication systems are completely disconnected and inoperable at the time of demolition start-up?
a. All fiber optic cables and lines will be removed by COMM. Contractor is responsible for demolishing/removing everything else.
28. If the contractor is responsible, will anything outside of the demolition sight be possibly affected or tripped?
a. No.
29. Will the contractor be responsible for plants dying as a result of roots being damaged that encroach upon the demolition site?
a. Yes. If contractor believes that a tree or shrub has been damaged, base arborist must be contacted immediately.
30. Does anything left in the building contain radioactive particles and was it tested for radioactivity?
a. Building has not been tested for radioactivity. CE Environmental will be able to test if contractor provides radiation meter.
31. Who will be responsible to terminate the fire main prior to demolition?
a. Contractor
32. Will all waste container contents need to be tested before leaving the Base? If so, how long will the contents be allowed to sit on Base to allow for testing? Who will be responsible for the testing?
a. Contractor must allow CE Environmental one week to test waste before leaving base.
33. Please define specifically what must be done to accomplish a desirable and successful monitoring well decommissioning for both wells to be removed.
a. MS licensed well driller will remove wells and use attached well decommissioning form to contact state.
34. Are there any specific drawings of the two wells to be removed in relation to their construction?
a. There are no drawings available for the wells.
35. How deep are the wells?
a. Approximately 20 -25 feet deep.
36. What is the specification for sealing off the wells?
a. Contractor will need to hire a MS licensed well driller to remove wells.
37. Will installation of backflush valves be necessary, or is the well to be 100% sealed?
a. Well is to be 100% sealed.
38. How much of the wells will need to come out?
a. The wells need to be removed in entirety.
39. What will be required to protect the existing third well that is to remain during both demolition of the building and decommissioning of the other two wells?
a. That well shall also be decommissioned and removed.
40. Will all HVAC components be considered Hazmat and/or a biohazard?
a. No.
41. Will all building components inside patient rooms need to be considered to contain bloodborne pathogens and treated as a biohazard?
a. Yes.
42. Does anything left in the building contain radioactive particles and/or was it tested for radioactivity?
a. Rooms have not been tested for radioactivity. All equipment has been removed.
43. Have the chillers and chill water been decommissioned with the water removed?
a. No.
44. Is there backflow prevention on the sewer or will we need to install those before demolition?
a. There is no backflow prevention on this sewer.
45. Is there a complete sewer plan of the premises?
a. There is none available for the building.
46. Who will be responsible to terminate the fire main prior to demolition?
a. Contractor
47. Will any operations on the base slow, halt or hinder the demolition and waste removal process besides the Special Olympics? If so, what will the duration of these activities be? Will there possibly be alternate routes to move materials and minimize delays?
a. No. Contracting will notify contractor of any unforeseen activity on base that will affect demolition.
Any time or material delays will be noted, and alternate route, if needed due to road closures, will be provided at this time.
48. Are the Monitoring Wells at the Dental Clinic polluted by Hazardous Chemicals?
a. No.
49. Plans sheet 5 of 6 has a note with an arrow stating "420 LF OF 4 FT WIDE CONCRETE SIDEWALK TO BE INSTALLED" The notes at the bottom of the sheet has a note stating "AT
AREA OF DEMOLISHED DRIVEWAY, INSTALL APPROXIMATELY 25 CY OF SELECT FILL TO
RAISE GRADE TO MATCH EXISTING NATURAL GROUND. CONSTRUCT FORMS AND POUR
APPROXIMATELY 420 LF OF 6 INCH HIGH 3000 PSI CONCRETE CURB. USE NO. 4 REBAR LAID
6 INCHES ON CENTER WITH 3 INCH COVER. TIE INTO EXISTING CONCRETE CURB" Are both the sidewalk and curb along Fisher Street to be removed and replaced, or just the sidewalk to be removed and replaced? There is approximately 60' ft of curb to be installed after the removal of the driveway connecting to Fisher Street.
a. Both sidewalk and curb must be constructed at area where driveway has been removed to connect east and west ends of sidewalk and curb into a continuous path.
50. Can the fill material underneath the structures be used for re-grading the site or will new fill material be required?
a. New fill material will be required.
51. What procedure is required for decommissioning the monitoring wells?
a. MS licensed well driller will remove wells and use attached well decommissioning form to contact state.
52. What is the depth of the well?
a. Approximately 20 - 25 feet deep.
53. Monitoring wells are typically used for hazardous waste - please confirm the wells do not contain any hazardous waste.
a. Monitoring wells were installed to monitor sanity sewer system. A decision document was based on sampling and determined that there was no threat to human health, and the well is closed.
54. Will the trees to be protected be identified by the base?
a. These trees can be identified by the base arborist for the contractor.
55. What procedure is required to protect the trees?
b. Contractor must maintain protective tree barriers. Base arborist will stop by periodically to check on trees.
56. Is there any medical or chemical hazardous waste on site?
c. Yes. The following hazards have not been removed from the site: amalgam, mercury, and human biohazardous waste.
57. Is there a base fluorescent bulb program?
d. Yes. Fluorescent bulbs will be dropped off at CE Environmental for recycling every Tuesday.
58. The SOW and Plans only address the removal of the street curbs and storm drains. There is no reference to filling or sodding those areas. The only reference to supplying fill and sod refer to the facility footprint and sidewalks. The following statement from the plans and SOW applies "At both facility footprints and demolished sidewalks, install approximately 3100 CY of select fill to raise grade to best fit natural ground. Compact fill to at least 85% standard proctor density. Slope to ensure positive drainage to prevent ponding water." Is the footprint left behind from street and storm drain removal to be filled and sodded?
e. There is no street or storm drain removal associated with the Demo Dental Clinic project.

Follow-up Question:
This task was not identified in the SOW. Please provide details of the underground storage tank. It is very difficult to provide a price without size, depth of tank, contents within tank, testing, etc. If no details are available, will a change order be considered once tank is uncovered?
Follow-up Answer:
Contractor must vacuum tanks and lines. Containment tank system consists of two 300 gallon steel tanks, 250 gallon steel vacuum tank, and pump not underground but inside mechanical room. Possible contents within tank have been identified and listed within a separate document named "Attachment 12 SDS index".

16. Has plumbing/sewer piping been included in any environmental testing?
Answer: No.
17. Has plumbing/sewer been tested for mercury and other heavy metals?
Answer: No.
18. Is there any possibility of other contaminates inside plumbing piping?
Answer: Yes.

Answer: Yes.
Follow-up Question:
This task was not identified in the SOW. Please provide details of the possible contaminates in plumbing/sewer piping. It is very difficult to provide a price without testing results of what contaminates if any are present and how much will need to be disposed of. Once the testing and results are completed, will a change order be considered?
Follow-up Answer:
Possible contaminants within sewer piping have been identified and listed within a separate document named "Attachment 12 SDS index". All residuals and debris will be disposed within a landfill certified to accept such material. Plumbing/sewer piping will not be treated as biohazardous.

32. Will all waste container contents need to be tested before leaving the Base? If so, how long will the contents be allowed to sit on Base to allow for testing? Who will be responsible for the testing?
Answer: Contractor must allow CE Environmental one week to test waste before leaving base.
Follow-up Question:
How many waste containers? This is unknown until testing and quantities of waste is identified. Once the testing and results are completed, will a change order be considered?
Follow-up Answer:
Estimated fifteen 55 gallon drums dependent on landfill restriction. The contractor is required to bid by weight of waste inside container rather than volume.

39. What will be required to protect the existing third well that is to remain during both demolition of the building and decommissioning of the other two wells?
Answer: That well shall also be decommissioned and removed.
Follow-up Question:
The SOW and drawings identified one (1) monitoring well requiring decommissioning. Please confirm the contractor will decommission three (3) monitoring wells at the northeast corner of the building.
Follow-up Answer:
Yes, three wells at the northeast corner of the building shall require decommissioning and removal.

41. Will all building components inside patient rooms need to be considered to contain bloodborne pathogens and treated as a biohazard?
Answer: Yes.
