ADDENDUM #1
The following sheet has been changed in the construction documents for the following project:
MAHG18-1093 Demo Dental Clinic
KEESLER AFB, MISSISSIPPI
Any information found within these documents shall take precedent over the information contained within the original construction documents. It is the responsibility of the contractor to review the documents and to include any revised or additional information in their proposal.
Changes to the statement of work:
Section 3.2.4, Bullet 8): Add new line items Bullet 8): “Contractor must vacuum tank and lines. Containment tank system consists of two 300 gallon steel tanks, 250 gallon steel vacuum tank, and pump not underground but inside mechanical room. Possible contents within tank have been identified and listed within a separate document labeled “SDS index”. All residuals and debris will be disposed within a landfill certified to accept such material.
Plumbing/sewer piping will not be treated as biohazardous.”
Section 3.2.4, Bullet 7): Delete sentence: “Decommission and remove old monitoring well at the northeast corner of the dental clinic building.”
Section 3.2.4, Bullet 7): Add new line items Bullet 7): “Decommission and remove three (3) monitoring wells at northeast corner of building.”
Section 3.2.4, Bullet 6): Add new line items Bullet 6): “All building components inside patient rooms will not be considered to contain blood borne pathogens and contain biohazardous waste.”
Section 3.2.4, Add new bullet item): “The following procedure shall be required to decommission and remove water wells as per Mississippi Code, Rule 2.14 Decommissioning of Abandoned or Unused Water Wells and Boreholes.
MS licensed well driller will remove wells and use attached well decommissioning form to contact state. See the following decommissioning procedures below:
The following procedures shall be followed in the decommissioning of any water well or borehole for which decommissioning is required under this regulation:
Grout for all holes shall consist of neat cement, cement grout, cement bentonite mixture (5-8% bentonite), or bentonite. Bentonite pellets may be added under free-fall conditions for depths not exceeding twenty-five (25) feet, providing pellets are placed in layers not more than five (5) feet deep and tamped into place after addition of each layer. Granulated or palletized bentonite may be placed to greater depths if introduced through a tremie pipe. Free-fall addition of other types of grout from the surface is prohibited. Neat cement grout bentonite mixture is a mixture of 94 lbs. of Portland cement, 5 lbs. of granulated bentonite, and 8 gallons of water.
(1) Obstructions shall be removed from the well casing;
(2) If there is reason to question the physical integrity of the well casing because of the age of the well or the material used for the casing, or there are no records to indicate that the annular space was grouted properly during construction of the well, the driller shall consult with MDEQ before plugging the well. In such instances, MDEQ may require that the casing be perforated to allow the introduction of grout into cavities or voids that may have formed outside the casing; or may require that the casing be removed from the hole prior to grouting;
(3) For abandoned water wells in agricultural fields, the casing shall be cut off and removed down to a minimum depth of three feet below land surface. After grouting, the excavation shall be filled with compacted soil.
In other areas, the casing shall be cut off and removed at least down to the ground surface elevation. MDEQ may authorize alternate methods of abandonment and/or abandonment by other than a licensed water well contractor, provided the results will meet the intent of the regulations. Only detailed written requests to utilize an alternate method of abandonment or to abandon a well without utilizing a licensed water well contractor shall be considered for approval. If approved, MDEQ will provide written authorization to the requestor.
(4) Abandoned water wells or boreholes shall be sealed from the bottom of the hole to ground surface using a grout as described in paragraph 1 above.
(5) MDEQ may authorize alternate methods of abandonment and/or abandonment by other than a licensed water well contractor, provided the results will meet the intent of the regulations. Only detailed written requests to utilize an alternate method of abandonment or to abandon a well without utilizing a licensed water well contractor shall be considered for approval. If approved, MDEQ will provide written authorization to the requestor.”
