PAST PERFORMANCE INFORMATION
(Section L Attachment 5)
Submit information on contracts that you consider relevant in demonstrating your ability to perform the Vance AFB BOS requirement. Include rationale supporting your assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining relevance, see Section M of the RFP. Note that the Government will not consider performance on an effort that concluded more than three (3) years prior to the issuance date of this solicitation. Offerors should identify which indicators of performance should be associated with the Technical Capability sub-factors on this acquisition:
(1) Sub-factor 1 – Technical Approach
(2) Sub-factor 2 – Mobilization Approach
(3) Sub-factor 3 – Program Management/Staffing Approach
(4) Sub-factor 4 – Performance Management/Quality Control
Offerors may include efforts accomplished to resolve problems encountered on the referenced contract as well as efforts to identify and manage program risk. Merely having problems does not automatically equate to a lowered confidence rating since the problems encountered may have been on a more complex program, or an Offeror may have subsequently demonstrated the ability to overcome the problems. The Offeror should clearly demonstrate management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified.
Provide the information requested in this submittal for each contract/program being described. Limit the number of efforts submitted to four (4) and the length of each submission to no more than seven (7) pages.
This cover page should be deleted when submitting the Past and Present Performance Information Sheet.

F. For each of the applicable sub-factors under the Technical factor in Section M “Evaluation Factors,” illustrate how your experience on this program applies to that sub-factor.
G. Quality and Satisfaction Rating: Provide any objective information (e.g., metrics, customer surveys, independent surveys) which demonstrates customer satisfaction with overall job performance and quality of completed service.
H. If applicable, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods etc.
I. Describe the nature or portion of the work on the proposed effort to be performed by the business entity being reported here. Also, estimate the percentage of the total proposed effort to be performed by this entity and whether this entity will be performing as the prime, subcontractor, or a corporate division related to the prime (define relationship). Ensure to describe how the experience of the past efforts can be reasonably attributed to the proposed effort, when the proposal clearly demonstrates the resources (e.g. financial resources, overall oversight and management, or other resources) of the parent or affiliated division will meaningfully affect performance of the instant acquisition.
