4. Printing shall be one and one-half spaced, single line spacing of titles only, with a font size no smaller than what is equivalent to Microsoft Word, Times New Roman, Font 11, with at least a one-inch margin on all sides of the printed page. Printing shall be on one side of page only.
5. The pages in each volume shall be numbered.
6. The offeror shall provide the digital (electronic) copies on CD-ROM disks in either Adobe Acrobat file format or Microsoft Word (version Office 2003 or lower) in read-only format. The Cost Model and any other supporting data which show the development and computation of rates and costs shall also be furnished in Microsoft Excel (version Office 2003 or lower) and include ALL cell formulas.
7. If the offeror submits proposal revisions, during or after discussions, any changed pages that are submitted shall be a different color (which will be designated by the contracting officer) than the original proposal pages, and shall have changed information clearly marked by a vertical line in the right margin of the page. The date of the revision and revision number shall be reflected on each changed page.
3.0 VOLUME 1 – PAST PERFORMANCE
3.1 General Information: Each offeror shall submit present and past performance information. This volume shall provide not more than five recent and relevant contracts per service area on which the prime offeror (including joint venture partners, individual companies which comprise the proposed organization, and/or critical and key subcontractors) has performed in the format requested below. This information, as well as other information the Government may obtain elsewhere, will be used to evaluate the offeror’s past performance. Offerors are notified that the Government will use the data provided in this volume that is determined to be recent and relevant to the effort at hand and data obtained from other sources in the development of a confidence assessment rating. The contracts listed may include those entered into with the Federal Government, agencies of state and local Governments, and commercial customers. If a service area or a significant portion of a service area is going to be performed by a subcontractor, the offeror is allowed a portion of the five contract references (noted above) for that service area to be for the subcontractor (no more than three references per subcontractor). If an offeror and its subcontractors do not have sufficient experience in a service area to list the required contracts, the offeror may include as part of the list, information regarding predecessor companies and/or proposed key personnel who will be involved in this contract. If the offeror is a combination of firms, information should be provided on contracts performed by the partnership, joint venture, or the individual companies which comprise the proposed organization.
3.2 Early Proposal Information: Submit the Past Performance Volume to the Contracting Officer at the address specified in Section L paragraph 3.10. The receipt of past performance information is subject to FAR 52.215-1(c)(3)(i), Late Proposals and Revisions.
3.3 Recent and Relevant Contracts: For purposes of this solicitation, recent contracts are defined as those where work was performed during the last three (3) years from issuance of RFP. A relevancy matrix (see Section J, Attachment 8) is provided for offerors to complete for each contract reference. The Government’s criterion for determining relevancy is found in Section M 4.2 Offerors are to note that the Government reserves the right to use both data provided by the offeror and data obtained from other sources. References for each service area shall include recent and relevant contracts with more than six months performance that involve the use of disciplines or capabilities similar to those necessary for performance of this requirement. Efforts
Amendment 0003 proposed have in fact been priced.
7. Common Costs - The offeror should carefully consider the Section B CLINs when proposing costs such as travel, see CLIN(s) X046, facility projects, see CLIN(s) X045, modernization and maintenance of Government-furnished Information Technology (IT) equipment, see CLIN(s) X043, and replacement of Government-furnished Base Operations Support equipment, see CLIN(s) X042 to avoid duplicating these costs in the transition or estimated costs.
8. Inventory –The offeror shall price and maintain an inventory of items necessary to satisfactory perform the requirement. The offeror is not required to propose for residual inventory upon completion of contract performance.
b. The Offeror’s cost proposal shall also include:
1. Name, address, phone, fax, and email of the prime and key and critical subcontractors’ cognizant DCAA offices. The Offerors cost proposal (hard copy and electronic) shall be simultaneously submitted to the cognizant DCAA office.
2. A completed Section B
3. A completed copy of the Manpower Matrix (provided in Section J, Attachment 6). The Manpower Matrix must be identical to the matrix provided in the technical proposal. The following total number of working man-hours per calendar year must be employed in filling out the Manpower Matrix and the Cost Model:

4. A completed Electronic Cost Model (provided in Section J, Attachment 7).
The offeror must prepare and submit proposed cost information on the referenced worksheets for all periods (mob/trans, basic, option years) by service area.
5. A consolidated listing of all disks and files submitted by the prime, joint venture partners, individual companies which comprise the proposed organization, and/or subcontractors per Section L, paragraph 7.1.2.a.6. above.
7.1.3 Financial Capability. In order to determine contractor responsibility the offerors shall explain how they plan to maintain adequate financial capability for a contract of this magnitude and length. Offerors must provide the last three years financial statements and Statements of Cash Flows for the firms or corporations proposed, and year-to-date financial information through the last quarter available. The year-end financial statements must be certified by an independent accounting firm, if practicable, or at least by an authorized officer of the organization.
