PERFORMANCE WORK STATEMENT
PERFORMANCE-BASED WORK STATEMENT (PWS)
FOR
Traditional Service Musician
DESCRIPTION OF SERVICES / GENERAL INFORMATION
1.1. Scope. The contractor provides keyboard playing (piano, organ, and electronic keyboard) to the Joint Base Andrews Protestant Traditional Service. The contractor performs music reflective of the nature of the Joint Base Andrews Protestant Traditional Service, primarily Protestant traditional and sacred music and hymns. The contractor accompanies congregational hymn singing. Additionally, the contractor provides keyboard playing to accompany the Protestant Traditional Service Choir. The contractor shall furnish all labor, tools, materials, and transportation to provide non-personal services for as the Protestant Traditional Service Musician in accordance with the PWS.
1.2. Services Description
1.2.1. Tasks contractor is responsible:
1.2.1.1. Work with the Service Chaplain to coordinate music performed for worship, special music programs, and activities at least one week in advance of performance.
1.2.1.2. The Protestant Traditional Service Musician will route all requests for equipment, facilities, and transportation through the Service Chaplain for approval and chapel staff coordination.
1.2.1.3. Must be able to work in harmony with Chapel staff, other contractors and volunteers.
1.2.1.4. In coordination with the Protestant Traditional Service Chaplain, provide services for regularly scheduled Protestant Traditional worship services at Joint Base Andrews Chapel 2 and for one (1) choir rehearsal per week. (The maximum number of units will not exceed 110: 52 units for worship services, 52 units for rehearsals, and 6 units for special services/events and rehearsals up to 2 hours each.
1.2.1.5. Personally perform the services described above or designate a substitute of equal or higher proficiency to perform the services. The name and qualifications of proposed substitute must be submitted to the Service Chaplain for approval at least 30 days in advance of a proposed absence. The Contractor is responsible for payment of the substitute.
1.2.1.6. The Contractor is accountable for security and cleanliness of all facilities utilized in conjunction with their programs.
1.2.1.7. Due to licensing laws, music contractors are required to purchase choir music through the Chapel Tithe and Offering Fund (CTOF). If distributing to choir members all Christian Copyright Licensing International (CCLI) guidelines must be followed.
1.2.2. Accountable for paying any Social Security payments, Federal or Local Income Taxes, and State Unemployment Insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract.
Contractor Requirements
1.3. General Information
1.3.1. Hours of Operation
1.3.1.1. Perform Sundays at the Traditional Worship Service at 0930 and music rehearsal at 1900 on Thursdays in Chapel Two.
1.3.1.1.1. Worship service and rehearsal times are subject to change based on the needs of the chapel community.
1.3.1.2. Perform additional worship services deemed necessary by the Service Chaplain for special worship and holiday services. The worship service takes place on Sundays at 0900. The rehearsal takes place on Thursdays at 1830.
1.3.1.3. Choir practices will be 2 hours, services will be 2 hours. Each rehearsal, service, special service or event is defined as one (1) unit.
1.3.1.4. Period of Performance (subject to contract solicitation date) with four (4) 12-month option periods.
1.3.2. Qualifications
1.3.2.1. Possess the ability to play various keyboard instruments: piano, electronic church organ and electronic keyboard.
1.3.2.2. Possess strong oral and written communication skills and be able to work with various chapel committees and groups.
1.3.2.3. Possess the ability to read music, including sight-reading.
1.3.2.4. Possess knowledge and experience in playing church music.
1.3.2.5. Participate in a qualification interview and an improvised music audition.
1.3.2.6. Furnish a resume including academic and music-related paid/volunteer work experience.
Dress and Appearance.
1.3.3. The contractor is required to dress in appropriate attire for worship services or other special religious observance. Tennis shoes, jeans or very informal attire is not acceptable.
1.3.4. Security requirements. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Government installation, shall abide by all security regulations of the installation.
1.3.4.1. Base passes. The contractor employees shall not enter any restricted areas unless authorize or escorted. No security clearance is required, but the contractor shall provide, on company letterhead, the names of each employee with the following information. This information shall be provided to the DD Form 577 holder for the 316th Force Support Squadron for approval and completion of a Long Term Contractor Access Pass. Once approved, the list of names shall be given to the Contracting Officer to put in the contract file. The contractor shall be responsible for the return of passes issued to employee(s) who separate from the contract before the expiration date of each contract period including any extensions to the contract. Lost or stolen passes shall be reported immediately to the contracting officer and the DD Form 577 holder with a written follow up within 72 hours. (Failure to properly account for passes may result in withholding contractor payment.)

1.3.5. Performance of services during crisis declared by the National Command Authority or overseas combatant commander. In the event the contractor is unable to perform his daily operations due to peacetime emergency and/or wartime contingency disturbances, etc., neither the Government nor the contractor shall take action for non-compliance with contract requirements.
1.3.6. Termination. The contract may be terminated in accordance with the termination clauses contained within the contract.
GOVERNMENT-FURNISHED PROPERTY AND SERVICES
2.1. GOVERNMENT FURNISHED SUPPLIES/EQUIPMENT
2.1.1. The contractor shall have access to the chapel administration equipment (i.e. copier, computers, fax machines, etc.) as required.
2.1.2. The contractor shall have graphic, reprographics and mailing support when approved by service chaplain.
2.1.3. The service chaplain will provide the contractor sheet music and music books.
Appendix 3.1.
SERVICES/PAYMENTS
3.1.1. Each service of worship is considered one unit for pay purpose.
3.1.2. Each choir/worship team rehearsal (Approximately 2 hours) is considered one unit for pay purpose:

THIS SECTION IS FOR SOLICITATION PURPOSES ONLY. THIS SECTION WILL BE PHYSICALLY REMOVED
FROM ANY RESULTANT AWARD, BUT WILL BE DEEMED INCORPORATED BY REFERENCE.
The following additions/changes are made to FAR Clause 52.212-1 which is incorporated by reference.
1. Period of Acceptance of Offers. The paragraph is tailored as follows:
“The offeror agrees to hold the prices in its offer firm for 120 days from the date specified for receipt of offers.”
2. Paragraphs e, and h of this provision are deleted from this solicitation.
ADDENDUM TO 52.212-1 PROPOSAL PREPARATION INSTRUCTIONS
a) INFORMATION REGARDING SUBMISSION OF PROPOSAL: This section specifies the format that offerors shall use in this Request for Proposal (RFP). The intent is not to restrict the offeror in the manner in which they will perform their work, but rather to ensure a certain degree of uniformity in the format of the response for evaluation purposes.
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The responses shall consist of two (2) separate parts; Part I
– Price Proposal, and Part II - Technical Capability.
B. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness.
C. Specific Instructions:
1. PART I – PRICE PROPOSAL – Submit one hard copy.
(a) Complete blocks 12, 17a, 17b, if applicable, and 30a, b, and c of the SF 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The SOLICITATION constitutes the model contract.
(b) Insert proposed unit and extended prices for each Contract Line Item Number (0001 thru 5001).
(c) Complete the necessary fill-ins and certifications in provisions. For the remainder of the RFP, except as elsewhere designated, the offeror shall submit only those pages that require a fill-in.
2. PART II – TECHNICAL CAPABILITY is the capability of the services being offered to satisfy the requirement of the Performance Work Statement (PWS) . The offeror’s response shall be limited to five
(5) pages, Times New Roman 12 font, double spaced, 8.5 inch x 11 inch paper. In addition, the offeror shall provide with his/her offer sufficient documentationin the form of a resume. The accompanying resume must be adequate to enable the Government to reasonably evaluate offerors ability to sucessfully perform the requirements of the PWS. The Government shall evaluate offeror’s total proposal for evidence of offeror’s demonstrated understanding of the requirements as stated in the
PWS.
After proposals have been evaulated, offerors may be scheduled for an interview to assess technical capability.
D. Documents submitted in response to this solicitation must be fully responsive to, and consistent with, the following:
1. Requirements of the solicitation (CLIN 0001 thru 5001), the Performance Work Statement (PWS), and Government standards and regulations pertaining to the PWS.
2. General Information:
Proposals shall be in the contracting office no later than the due date and time specified in this solicitation.

52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable (LPTA)
(1). PART I – Lowest Price Technically Acceptable (LPTA) – an award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors.

(2). PART II – Technical Approach – is the capability of the services being offered to satisfy the requirement of the Performance Work Statement (PWS) . The offeror’s response shall be limited to five (5) pages, Times New Roman 12 font, double spaced, 8.5 inch x 11 inch paper. In addition, the offeror shall provide with his/her offer sufficient documentation in the form of a resume. The accompanying resume must be adequate to enable the Government to reasonably evaluate offerors ability to sucessfully perform the requirements of the PWS. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph
(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days.
52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 15 days;
provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 66 months.

(a) Definitions. As used in this clause--
Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.
Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow
(WAWF).
Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.
(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.
(c) WAWF access. To access WAWF, the Contractor shall--
(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and
(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site.
(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF.
