1.0 INTRODUCTION:
This project will provide an asphalt-paved road surface for Enoch from Sputnik Street to the
Base-X area, a distance of approximately 3,500 feet. The proposed pavement section is based on the geotechnical engineering recommendations contained in the attached geotechnical report.
The proposed pavement section will consist of 4 inches of hot-mix bituminous asphalt concrete over 12 inches of aggregate base course. The existing asphalt millings can be re-used as a part of this project. However, the diameter of the existing millings is too variable to use as base course unless it is processed and mixed with new base course material. All pavements and aggregate base course will be designed and installed in accordance with CDOT standards and specifications. All work to be conducted under this contract shall be done under the general conditions of the Statement of Work (SOW) contained herein and all applicable specifications and attachments.
The existing project site consists primarily of the existing Enoch Road gravel road section and associated roadside ditches. The surrounding area is undeveloped and consists of native groundcover and a few trees. Existing utilities are present along and through the Enoch Road corridor and consists of: drainage culverts, overhead power lines and poles, sanitary sewer mains and manholes, underground telephone lines, and underground fiber optic lines. Cherokee Water
District has a sanitary sewer metering facility located on the south side of Enoch about 1,000 feet east of Sputnik Street. There is an existing gravel access road connecting this facility to Enoch.
There are also existing roadway signs along Enoch as well as guardrails on either side of the road at the concrete box culvert crossing location (approximately 200 feet east of Sputnik Street).
The proposed pavement section will be crowned at the centerline with a typical cross-slope of
2% draining to either side of the roadway. This typical section will transition to and from a super-elevated cross-section where Enoch takes a 90-degree bend to the south. The overall paved area will be 26 feet in width. Two 12 foot travel lanes will be provided. Solid double yellow centerline will separate opposing travel directions and solid white lane edge striping will be provided on either side of the pavement to frame the travel lanes. Signage along Enoch will consist of either re-furbished/re-installed existing signs or new signs. All signage and striping will be installed per CDOT and MUTCD standards and recommendations.
The vertical alignment of the new roadway will follow existing grades as closely as possible, factoring in the proposed depth of the pavement section. This will mitigate the need for significant import or export of material to/from the site, excepting engineered aggregate base course. It will also mitigate the need to extensively grade into the existing areas adjacent to the
Enoch Road cross-section.
Design criteria for the new roadway was obtained from the AASHTO “Green Book” as indicated in the references section and as referenced in the CDOT design criteria. A classification of “local rural road” was used as general guidance for the design. A local rural road with a design speed of
30 mph can support up to 400 vehicles per day on “rolling” terrain and this classification was used for the project. Design values for maximum grades (7%) as well as K values for sag and crest vertical curves (19 and 37, respectively) were incorporated into the vertical design of the project. The minimum width of traveled way, as well as width of graded shoulder, also meets or exceeds the AASHTO requirements.

There are three existing drainage culverts which cross the Enoch Road alignment. All three are located on the east-west leg of Enoch to the east of the Sputnik Road intersection. This project is proposing to replace the two westernmost culverts with Class IV reinforced concrete pipe (RCP) near their original locations and of the same diameter as the existing pipes. The easternmost culvert, near the 90-degree curve to the south, is proposed to be removed and not replaced.
Drainage tributary to this area in the existing condition will now travel via roadside ditch to the new culverts west of this location. The new culverts were analyzed with respect to minimum cover beneath the pavement section. ASTM C 76 was utilized to calculate minimum cover and it was found that the required cover over Class IV RCP, based on the project requirements, was
18” between the bottom of HMA and the top outside face of the pipe. This parameter was then used in the vertical design of the new roadway, factoring in the highest point of the pipe and the lowest point of the HMA occurring along the northern edge-of-asphalt.
The existing guardrail mentioned above has been evaluated against CDOT standards and has been found to be deficient in at least one criteria. Based upon review of the CDOT M&S standards, the minimum length of a guardrail spanning a box culvert/headwall/wingwall installation is 37’-6” + 12’-6” + projected width of wingwall-to-wingwall dimension + 12’-6” +
37’-6”. For the northern guardrail, the required overall rail length would be roughly 120 feet. For the southern, roughly 137 feet in length. In the existing condition, the northern guardrail is approximately 40 feet in length and the southern guardrail is approximately 53 feet in length with both dimensions below the minimum length as determined by CDOT criteria. For this reason, it is suggested that as a part of this project the guardrail be removed and replaced to
CDOT parameters.
2.0 BACKGROUND:
The 50th Civil Engineer Squadron at Schriever AFB is seeking proposals to reconstruct Enoch
Road from Sputnik Street to the Base-X Area, a distance of approximately 3,500 linear feet.
This section of roadway is currently not paved. Asphalt grindings have been placed to help stabilize the roadway and to reduce dust. Paving this section with asphalt pavement will provide a more durable roadway surface and will reduce maintenance. The project will provide a paved roadway from the base mainside to future developments in the Base-X area.
3.0 SCOPE OF WORK:
The Contractor shall provide all labor, tools, and materials to construct new asphalt pavement and incidental improvements in strict accordance with this SOW at Schriever AFB, CO. The
Contractor shall provide the Government with complete construction, and warranty for this project.

4.0 DESIGN REQUIREMENTS
The contractor shall complete the work in accordance with the Project Drawings included herein.

5.0 WORK REQUIREMENTS
5.1 GENERAL PROVISIONS
5.1.1 The Contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting SAFB
Specifications.
5.1.2 All work must conform to the Project Drawings and Specifications, unless specific instructions contained herein exceed the requirements of the specifications.
5.1.3 The work area will be confined to the applicable areas as described in this document
5.1.4 A Form 66 Submittal Register is provided for this project. Provide submittals and maintain the Submittal Register throughout project construction period of performance.
This list of submittals is not all-inclusive; other submittals and forms may be required.
Submittals shall be in accordance with Section 01 33 00, Submittal Procedures, of the
Specifications.
5.1.5 Before project commencement & closeout, ensure that all Environmental reports have been submitted and approved by the CO and environmental group (CEIE) in SAFB Civil
Engineering.
5.1.6 The contractor shall promptly clean up any spills and tracked or spilled soils or debris.
The contractor shall promptly clean up all work areas, restore pavements, landscaping, GLEN 15-4104 Page 6 and irrigation in kind, and repair any damaged real property after work activities have ceased.
5.1.7 Compliance with the US Access Board’s Architectural Barriers Act standards shall be maintained at all times. Any disruptions to traffic flow shall be coordinated with the CO and detailed traffic control plans conforming with the Federal Highway Administration’s
2009 MUTCD.
5.1.8 DAMAGES TO GOVERNMENT BUILDINGS, EQUIPMENT, OR VEGETATION:
The contractor shall take appropriate action to restore any contractor caused damage to existing property or services, whether government, private, or commercially owned.
The contractor shall use reasonable care to avoid damaging buildings, equipment or vegetation. The contractor shall be liable for any damages to buildings, equipment or vegetation. The contractor shall replace, repair or pay for any damages as determined by the CO.
5.2 PROJECT REQUIREMENTS
5.2.1 Remove existing asphalt grindings and subgrade material.
a. Remove existing asphalt grindings and subgrade material to the limits and depths shown on the project drawings, and properly dispose of unsuitable materials off of the installation. The existing asphalt millings can be re-used as a part of this project.
However, the diameter of the existing millings is too variable to use as base course unless it is processed and mixed with new base course material. All removals shall be done to a straight and clean sawcut line. Sub-grade and base materials shall be removed to a sufficient depth to allow for the full depth of asphalt, and new base material to be placed. Weight tickets shall be provided for all removed materials.
b. Scarify and compact the sub-grade material to optimal compaction, correcting moisture as needed. Employ artificial heating, if needed, to properly consolidate and compact the subgrade prior to placement of new base course. Provide ASTM soil compaction tests at a minimum of twelve (12) random and representative locations in the subgrade. Compaction test results shall be provided to the contracting officer within three (3) calendar days of the test.
5.2.2 Place and Compact Base Course
a. Place clean CDOT Class 6 aggregate base material and compact to optimal compaction, correcting moisture as needed. Base course shall be placed and compacted in lifts not exceeding three (3) inches. Provide ASTM soil compaction tests at a minimum of twelve (12) random and representative locations in each lift.
Compaction test results shall be provided to the contracting officer within three (3) calendar days of the test.
b. Weigh tickets are required for all aggregate base deliveries, and shall be collected by the project QC representative and delivered as a submittal to the contracting officer.
5.2.3 Placement of Hot Mix Asphalt Pavement
a. All Hot Mix Asphalt (HMA) materials or work will be evaluated for conformity to the Contract in accordance with subsection 105.05 of the Colorado Department of

Transportation (CDOT) Standard Specifications for Road and Bridge Construction
(Latest Edition). No lift shall exceed 3 inches. Total depth of HMA shall be 4 inches.
b. Hot Mix Asphalt (HMA) mix designs shall be specified as part of the project design.
All asphalt applications shall be per Colorado Department of Transportation (CDOT)
Standard Specifications for Road and Bridge Construction (Latest Edition) SECTION
702 - BITUMINOUS MATERIALS. Superpave Performance Graded Binders shall conform to the requirements listed in Table 702-1. Binder Grade for HMA Mixtures shall be PG 64-22 for the base course and PG64-28 for the upper course unless otherwise approved by the government.
c. A tack coat shall be applied on all vertical surfaces and between asphalt layers. A prime coat shall be applied between the aggregate base and the asphalt base course.
Tack coat shall be in accordance with Section 407, Prime Coat, Tack Coat and
Rejuvenating Agent of the CDOT Standard Specifications.
5.2.4 Install Signs and Paint Markings
a. All damaged and removed striping shall be restored in kind. Paint shall be applied to clean, dry surfaces and only when the air and pavement temperature are above 50°
Fahrenheit and less than 95° Fahrenheit. Paint temperature shall be maintained within these same limits. Paint shall be applied pneumatically with approved equipment rate of coverage specified. The contractor shall provide guide lines and templates as necessary to control paint application. Special precaution shall be taken in marking numbers, letters, and symbols. Edges of markings shall be sharply outlined. The paint shall be homogeneous, easily stirred to a smooth consistency, and shall show no hard settlement or other objectionable characteristics during a storage period of 6 months.
Paint for Roads and Streets shall conform to FS A-A-2886, FS TT-P-1952 or High
Build Acrylic Coating (HBAC), color as indicated/selected, color as indicated.
b. Two (2) coats of paint shall be applied.
c. Pavement marking paints shall comply with applicable state and local laws to ensure compliance with Federal Clean Air Standards. Paint shall conform to the restrictions of the local Air Pollution Control District.
i. Certificate needs to be submitted stating that proposed pavement marking paint meets the Volatile Organic Compound (VOC) regulations of the local
Air Pollution Controlling District having jurisdiction over the geographical area in which the project is located.
ii. Colorado Department of Transportation (CDOT) Standard is allowable as an alternate pavement marking specification. Submittal must be approved by the
Contracting Officer.
d. Waterborne & Durable Waterborne Paint:
i. Reflective markings: Pigmented binder shall be applied evenly to the pavement area to be coated at a rate of 105 plus or minus 5 square feet per gallon (each coat). Glass spheres shall be applied uniformly to the wet paint on road and street pavement at a rate of 7-8 lbs per gallon of paint. Minimum thickness of all striping shall be 25 mil.

e. Signs: Sign reflectivity and heights shall conform to MUTCD and CDOT standards.
The contractor shall fabricate the signs in accordance the most current version of
Colorado Department of Transportation Standard Specification for Road and Bridge
Construction, the Standard Alphabets for Highway Signs, and Standard Highway
Signs as published by the FHWA, and the Colorado Supplement to Standard
Highway Signs. The backside of all signs will have “Schriever” brown backing that matches existing Schriever AFB signage. Apply a durable preprinted label on the back of the new signs that displays the year and month the signs were installed.
Artwork and sign details shall be submitted for government approval. Sign posts shall be set in a 12” diameter by 3’ deep concrete footing. The upper post shall be a
2”x2” perforated galvanized square tubing (12 gauge). The lower post shall be a 2 ¼” x 2 ¼” perforated galvanized square tubing (12 gauge) which is embedded in the concrete footing. Two 3” x 3/8” bolts with locknut (zinc plated) shall secure the upper post to the lower post.
5.2.5 Guardrails
a. The existing guardrails shall be modified as shown on the project drawings. The guard rails shall be installed in accordance with CDOT Standard Plan M-606-1 and
Division 606 – Guardrails of the Standard Specifications for Road and Bridge
Construction.
5.2.6 Traffic Control.
a. Enoch Road shall not be closed during construction. At a minimum, one lane of traffic in each direction shall remain open at all times. All side streets and driveways shall remain accessible during construction. The contractor may anticipate that there will be minimal traffic, but access must be provided to, at a minimum, emergency vehicles, Security Forces and Cherokee Metropolitan Water District staff.
b. Prior to commencement of work, the Contractor shall submit a Traffic Control Plan, and a Method of Handling Traffic, for each phase of construction.
c. The Traffic Control Plan shall include any and all required advance signage, channelizing devices required, barricades, flaggers, shuttle bus detour and/or alternate turn-around routes, and a means for inspecting and maintaining traffic control during off hours.
d. Traffic control plans shall also include installation of any and all temporary fencing, and limitations affecting pedestrian traffic.
e. If demolition or construction operations pose a safety hazard to pedestrians, handicap access and/or vehicles in the vicinity, appropriate measures will be taken to eliminate the hazard.
f. Suitable warning signs shall be placed near the beginning of the worksite and well ahead of the worksite for alerting approaching traffic from both directions. Small markers shall be placed along newly painted lines or freshly placed raised markers to control traffic and prevent damage to newly painted surfaces or displacement of raised pavement markers. Painting equipment shall be marked with large warning signs indicating slow-moving painting equipment in operation. The Manual on
Uniform Traffic Control Devices (MUTCD) for Streets and Highways and local jurisdictional requirements shall be used for guidance for traffic control procedures.

g. The Contractor shall prevent traffic from crossing a wet stripe. Stripes which have been marred or picked up by traffic before they have dried shall be repaired at the
Contractor’s expense. Removal of the paint from the vehicles that crossed wet paint shall be at the contractor’s expense.
h. A two week notice will be given and approved by the CO before closing off any section of the roadways.
i. All traffic control plans including pedestrian traffic will be approved by the CO prior to implementation.
5.2.7 Utility Locates.
a. A Form 103 BCE Work Clearance Request form is required prior to execution of any work. The 50 CES Project Manager will initiate and process the 103 Work Permit.
The Contractor shall be responsible for contacting UNCC Locates at 1-800-922-1987 and obtaining mark-outs and clearances from Commercial Utility Companies
(Telephone, Gas, Electric, Water and Sewer). The contractor shall provide proof from the respective utility company that the site is cleared for construction. The Contractor shall be responsible to preserve any and all utility locate markings, and is ultimately responsible for identifying and avoiding contact with existing utilities. This shall include taking any and all additional measures to ensure that utility locate markings are preserved as best field management construction practices allow.
b. All Schriever AFB utility locates will be accomplished using best available information including: pavement markings, installation utility maps, and as-built drawings.
c. It is the Contractors responsibility to be aware of utilities within the project area. The
Contractor will be responsible for damages caused to base infrastructure utilities as a result of construction activity.
d. The Contractor shall, upon any and all breaches of base infrastructure utilities, notify the base fire department or appropriate utility of any immediate hazard, critical leak, or outage caused as a result of construction.
5.2.8 Project Schedules and Updates.
a. The contractor shall be required to submit weekly project schedule updates. Project updates shall clearly note project progress, project time count, project milestones, material lead times, and tentative/adjusted finish dates. A two week look ahead schedule shall be submitted with the weekly project schedule update.
b. Items to be included in the contractors’ progress schedule include, but not limited to:

5.2.9 Materials Testing/Quality Assurance
a. The contractor will be required to secure and coordinate all project related materials testing to include in-situ soil density testing and compaction testing. Concrete testing shall be per ASTM standards, and include, as a minimum, air content, temperature, slump, and compressive strength. The 50 CES construction inspector shall be given the opportunity to accompany the contractors’ representative to observe all materials testing practices.
b. All completed layout, test submittals, and project schedules shall be reviewed internally for completeness prior to submittal to Schriever AFB 50 CES Engineering.
5.2.10 Additional services incidental to the work including, but not limited to:
a. Construction surveying/layout.
b. Installation and removal of all temporary fencing as needed to secure the construction work zones.
c. Protection of existing irrigation. If irrigation equipment is encountered and is in conflict with the items of work, the irrigation equipment shall be relocated and/or replaced in kind, to accommodate the work.
d. Backfill and compaction. All curbs, gutters and excavations shall be backfilled and compacted with select fill.
e. Erosion Control. The Contractor shall provide erosion control during construction in accordance with the Project Drawings and Specifications. The erosion control measures shall remain in place until the permanent control is established. The contractor shall be responsible to remove all temporary erosion control materials.
f. Final project clean up.
g. Submittal of “as-built” drawings and close out submittals per Form 66 requirements.
h. All other items incidental to completion of the work in its entirety.
i. The Contractor shall return all areas affected by construction activities, both within and outside the scope of this project, to their previous existing condition, to include site cleanup, repair of damaged real property, restoration and establishment of native vegetation, and removal of any and all construction refuse generated.
5.2.11 Construction Water. All construction water shall be obtained from the water fill station at Building 826. 50 CES will provide an access code and instructions for using the water fill station. The water fill station will meter the amount of water used. Fire hydrants shall not be used unless expressly authorized, in writing, by the Contracting Officer.

The CDOT Standard Details included in the Project Drawings are included for the contractor’s convenience. All work shall be accomplished in accordance with CDOT Standard Specification for Road and Bridge Construction. Other applicable CDOT Standard Drawings not included in the Project Drawings are required
5.4 SPECIAL CONDITIONS/REQUIREMENTS
5.4.1 A Bid Tabulation Form is included. This bid tabulation lists the only pay items to be used for this contract. If the contractor believes that additional line items are necessary for completion of the required work, they may add them to the proposal sheet and attach a clear explanation for the additional line item(s). The contractor is to propose the quantities/amounts to cover all costs to complete the project. The proposal sheet will be used by the Government for purposes of evaluation of the proposal(s).
5.4.2 All construction work shall be subject to stoppages and delays due to critical SAFB mission requirements.
5.5 GUIDES & REGULATIONS
5.5.1 The construction specifications for this contract are the Colorado Department of
Transportation (CDOT) 2011 Standard Specifications for Road and Bridge Construction, with all published Standard Special Provisions, with the following exceptions: Sections
100, 102, 103, 104, 105, 107, 108, and 109. Division 01 – General Requirements are provided as attachments to the Statement of Work, as follows:

5.5.2 The project shall conform to all applicable Federal, State and local laws, regulations and base-specific standards and applicable Unified Facilities Criteria (UFC). Where there is a conflict between the referenced standards, the most stringent shall take precedence. All work accomplished by the Contractor shall comply with these criteria unless a waiver is obtained from the Air Force when conditions warrant such consideration.
5.5.3 The Contractor must comply with the SAFB Overarching Environmental Specifications, which outlines provisions to help ensure Contractor compliance with environmental law.
Include a Spill Prevention and Response Plan with every project. See Section 01 57
20_00 10 of the Specifications.
5.6 AS-BUILTS & DRAWINGS
5.6.1 GIS and CAD Requirements

All GIS deliverables will be provided in an ArcGIS 10.x geodatabase using the SDSFIE 3.0

Any alterations or additions to the SDSFIE standards need to be approved prior to delivery of the personal geodatabase by the GeoBase Program Manager at Schriever AFB. The GIS data delivered will need to be projected to both NAD 1983 State Plane Colorado Central FIPS 0502
US Feet and Geographic Coordinate System WGS 84. Duplication of this data in both coordinate systems assures compliance with both DoD and State standards.
Common Installation Picture (CIP)
The CIP is the standard USAF geospatial data set that depicts facilities and physical features on an installation. The CIP is comprised of georeferenced vector layers in a digital format with related attribute information.
CIP vector layers shall include those features that generally depict installation facilities and physical features including, but not limited to: structures, paved surfaces, improved surfaces, hydrography, and boundaries.
Mission Data Sets (MDS)

MDS is the standard USAF geospatial data set that depicts thematic mapping layers such as utility systems or environmental features on an installation. Examples of utility systems attribute fields include, but are not limited to manufacturer, year installed, depth, disposition, use, size, material, and condition.
CAD - Civil Drawings
All CIVIL drawing data will comply with the AEC CAD Standard Release 5.0 currently being used by Schriever AFB, Civil Engineer Squadron. All as-built drawing shall be delivered on a
CDROM/DVD in both *.PDF and *.DWG format. All sheets should be converted to PDF by their discipline. All DWG files should be projected to both NAD 1983 State Plane Colorado
Central FIPS 0502 US Feet and Geographic Coordinate System WGS 84. Duplication of this data in both coordinate systems assures compliance with both DoD and State standards. All civil drawings shall clearly indicate survey site control monuments for basis of bearings and distance.
The *.DWG file delivered shall be created using AutoCAD Map 3D 2014. Accuracy level of data provided shall be +/- .02 feet. All Civil drawings shall reflect mean combination ground to grid factor, be printed upon any calculation sheets anytime ground (modified) State Plane
Coordinates are used, and be noted in the bearing source statement. Digital media must have an external label listing the name of the project and contract number and the words “As-Built
Record Set”. The *.DWG’s shall be bound and have no external references. When opened the
*.DWG’s shall be seen exactly as they should be plotted. Include any symbol libraries, font libraries, color tables, pen tables, plot configuration files, user command files, etc. necessary for a complete project. Compress, reduce, and/or purge all files using the appropriate utilities. All
*.CTB and *.PC3 files shall be supplied if used on the project.
6.0 PERFORMANCE PERIOD
A Period of Performance of Ninety (90) Calendar days will be allowed from the execution of the
Contracting Officer’s (CO) Notice To Proceed (NTP). The Period of Performance includes processing all required submittals, installation of all components, testing, submission of as-built drawings, and lead time for any ordered materials. The Performance period includes twenty one
(21) calendar days for approval of all preliminary submittals, plans, and permits.
Any and all project delays shall be addressed on an as-needed basis, and any requests for contract time extension shall be provided, in writing, to the contracting officer for consideration by the 50 CES project manager.
If the work period extends into periods of unsuitable weather, the project may be suspended for
Winter Exclusion.
6.1 WORK HOURS
Standard work hours under this contract are Monday through Friday, 7:00 a.m. to 4:00 p.m.
Mountain Standard Time (MST). Upon written request from the Contractor, the CO may grant permission to deviate from the standard work hours on a case by case basis. Request to the CO for non-standard work hours shall be requested at least 48 hours in advance. In addition to non-standard work hours, the Contractor shall not work on federal holidays without written approval from the CO.

7.0 MEETINGS
The time and place of meetings shall be determined by the CO. Project meetings shall include, at a minimum, the contracting specialist and the Project Manager.
7.1 KICK-OFF MEETING
The contractor shall attend one pre-construction (kickoff) meeting at Schriever AFB. B. A pre-construction meeting will be held at the Contracting conference room within two (2) weeks after the award.
7.2 PRE-CONSTRUCTION SITE SURVEY
The Contractor, prior to the start of any on site construction activities, shall conduct a condition survey with the Contracting Officer and the 50th CES Project Manager, after which the
Contractor shall note the condition of trees, shrubs, grassed areas, concrete, and any structural damage that may exist immediately within or adjacent to work site.
7.3 PROGRESS MEETINGS
Weekly progress meetings shall be held. The contractor shall provide the weekly project schedule updates prior to the weekly progress meeting for discussion. These meetings will cover the progress made in the past period and will include any pertinent issues which could affect future contract progress. The Contractor will be responsible for providing Progress Meeting
Minutes within 2 business days of conclusion of the meeting.

a) Meeting Attendees and Contact Information
b) Project Percent Complete (Actual and Scheduled)
c) Schedule
d) Two Week Look Ahead
e) Material Submittals and Statuses

8.0 CONTRACT COMPLETION
8.1 DELIVERY & WARRANTY
The Contractor shall complete all requirements prior to final inspection. Following final inspection, all drawings, O&M Manuals, and CDs shall be delivered to the CO within 14 calendar days.
8.2 PRE-FINAL INSPECTION
The Contractor shall conduct a pre-final walk through inspection with Base personnel and publish the pre-final inspection findings in a pre-final inspection (punch list) report. The
Contractor shall include a draft DD Form 1354, Transfer and Acceptance of Real Property to the
Contracting Officer for review.

8.3 FINAL INSPECTION
The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punch list) report has been resolved. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of
Real Property to the Base Civil Engineer (BCE) or other appropriate organization for signature and acceptance, if required.
Final acceptance: After final “as-built” drawings have been submitted and approved by the CO, and approved site clean-up the contractors work will be complete.

APPENDICES
Appendix 1: DELIVERABLES
The Contractor shall provide submittals to the CO via the IMT Form 3000. A complete list of deliverables needed for approval can be found on the Project Submittal Form AF-66. Any submittal that will be in electronic format shall use a version that is compatible with the
Government’s 2010 version of Microsoft Word, Excel, Adobe Acrobat, and AutoCAD. All deliverables shall also be dated.
Task Project Work Plan (Deliverables)
The Contractor shall produce and submit a project work plan for approval. The plan shall contain at a minimum the strategy and milestones for completing the project tasks and completed in Gant Chart Format.
CONTRACT PROGRESS SCHEDULE (AF 3064)
The Contractor shall submit a signed AF 3064 Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance. The
Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance.
The PM and CO shall approve the first (baseline) progress schedule and it shall not be altered without the approval of the CO.
The PM and CO shall sign subsequent and periodic updates to the progress schedule.
CONTRACT PROGRESS REPORT (AF 3065)
The Contractor shall submit AF 3065 every two weeks after the issuance of NTP. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. If the Contractor is more than 5% behind schedule, the Contractor shall note in the Remarks section the reason why and what is being done to get back on schedule. The PM will sign the AF 3065 and send to the CO for coordination and approval.
An AF 3065, signed by the PM and the CO, shall accompany each invoice submitted in WAWF.

Appendix 2: Form 103 WORK CLEARANCE PROCESS
A Form 103 BCE Work Clearance Request form is required prior to execution of any work. The
50 CES Project Manager will initiate and process the 103 Work Permit. The Contractor shall be responsible for contacting UNCC Locates at 1-800-922-1987 and obtaining mark-outs and clearances from Commercial Utility Companies (Telephone, Gas, Electric, Water and Sewer).
The contractor shall provide proof from the respective utility company that the site is cleared for construction. The Contractor shall be responsible to preserve any and all utility locate markings, and is ultimately responsible for identifying and avoiding contact with existing utilities. This shall include taking any and all additional measures to ensure that utility locate markings are preserved as best field management construction practices allow.
Appendix 3: SAFB SECURITY REQUIREMENTS
Contractor is required to provide identification badges for their employees. All Contractor personnel shall wear these badges while on SAFB. Badges are required to identify the individual, company name, and be clearly and distinctly marked as Contractor. These are not substitutes for visitor, temporary or any other SAFB generated badges.
The contractor shall comply with all SAFB security requirements. SAFB is a controlled/restricted area and special security procedures are utilized to deter espionage, sabotage, and theft of Air Force resources. The contractor shall be responsible for security of his work areas in cooperation with applicable base agencies.
The contractor shall appoint, in writing to the Government, a Security Representative to arrange all needed security support for this Performance Work Statement (PWS). The contractor shall arrange all needed security support for this project. Contractor shall coordinate security matters with the 50 Civil Engineer Squadron (50 CES) and other base agencies, as needed. Assistance from the 50 CES Security Manager and 50 Security Forces Squadron (SFS) personnel will be provided to the contractor’s Security Officer.
It is the sole responsibility of the Contractor to obtain any and all required escorts prior to starting work.
Photographs will not be taken on Schriever AFB without prior coordination with the Security
Manager and approval of the commander. Upon approval, photographs shall not be taken of any mission equipment, classified or sensitive materials. Use of digital cameras is limited and a make, model, and serial number of the camera to be used must be supplied to the Security
Manager for approval prior to use.
Incidents of a violating photography procedures shall be treated as a breach of security and that person shall be susceptible to apprehension, search, confiscation of photography equipment and materials, and detainment. The contractor shall insure personnel are briefed on base security requirements.

Appendix 4: RESTRICTED AREA PROCESS
PLEASE NOTE THAT ALL REQUESTS FOR BASE AND RESTRICTED AREA
ACCESS REQUIRE 3 BUSINESS DAY ADVANCE NOTIFICATION OF THE
SECURITY MANAGER.
STEP 1 – Contractors must have visited Pass and ID in the Visitors Center and obtained a
Visitors Badge, or “V” badge in order to obtain temporary base access to retrieve their
“Schriever Form 75” access pass.
Your completed access pass will allow construction crews base access for construction purposes without visiting Pass and ID to obtain visitors badges.
THIS IS NOT THE SAME AS ACCESS TO THE RESTRICTED AREA. Security
Badges are required to enter the Restricted Area, and the process is outlined below.
STEP 2 – Fill out the attached “Contractor/Vendor Base and/or Restricted Area Access Request
Form” and send it via E-Mail to the Security Manager, TSgt Larry Mackey, 567-3712, at larry.mackey@us.af.mil.
The completed form will be coordinated by the Security Manager and forwarded to 50 Security
Forces Squadron Pass and ID for processing.
STEP 3 – After request forms are validated and processed by Security Forces, passes are generated and sent to the West Gate Inspection Location for Contractor pick up. Please allow 3 business days for pass processing.
STEP 4 – Contractors may obtain passes and gain access to the base the same day.
If entering the base after obtaining an access pass, vehicles will be inspected prior to entering the gate.
If the Contractor requires access to the Restricted Area (RA), Contractor must proceed to the
West Entrapment Gate to enter the RA where the vehicle will be searched again.

Appendix 5: SECURITY BADGING PROCESS
PLEASE NOTE THAT THE SECURITY CLEARANCE AND BADGING
PROCESS WILL TAKE 3 TO 4 WEEKS.
STEP 1 – Fill out the “Standard Form 85P Questionnaire For Public Trust Positions” and submit to the 50 CES Security Manager through either the Project Engineer, or the Contracting Office.
It is the responsibility of the Contractor to obtain contact information for the Project
Engineer and the Project Contracting Officer. All contact information should be obtained at the time of contract award.
• Keep a copy of the Standard Form 85P, it will help you in Step 3.
STEP 2 – Upon completion of the “Standard Form 85P”, the Contractor will be asked to fill out a form “AF IMT 2583 Request For Personnel Security Action”.
Fill out Blocks 1 through 7; and Block 30. In Block 30, make sure to include a valid E-
Mail address.
STEP 3 – The Contractor will be notified via E-Mail by the 50 SW/IP Office, that an investigation has been initiated.
• The E-Mail received from the 50 SW/IP has the contact information for the 50
SW/IP office.
• Carefully read the attachments and log in to the EQIP Website to begin the clearance paperwork
• YOU HAVE 30 DAYS TO COMPLETE THE ONLINE CLEARANCE
PROCESS FROM THE TIME YOU ARE NOTIFIED BY 50 SW/IP THAT AN
INVESTIGATION HAS BEEN INITIATED.
• IF YOUR CLEARANCE PROCESS IS NOT COMPLETED WITHIN 30 DAYS,
A STATUS REVIEW WILL BE CONDUCTED AND THERE IS A CHANCE YOUR
CLEARANCE WILL NOT BE GRANTED.
• No additional project time will be allotted due to incomplete security clearance paperwork.
STEP 4 – Contractor should complete the online EQIP Form. * It is important to click
“RELEASE” to send the completed form back to the 50 SW/IP Office. *
• Print the 2 signature pages for your records.
• You will be notified via E-Mail by 50 SW/IP to set up a fingerprinting appointment

STEP 5 – Set a fingerprinting appointment. After setting your appointment, contact the Project
Engineer or the Contracting Office; to make them aware of the date and time access to Schriever
AFB will be required for your appointment.
• Fill out the “Application For Federal Employment Form OF612”, and the
“Declaration For Federal Employment”, and bring them with you to your fingerprinting appointment.
STEP 6 – After your fingerprinting appointment, contact the 50 CES Security Manager, TSgt
Larry Mackey, 567-3712, at larry.mackey@us.af.mil to set up a time to review security process and obtain badging paperwork, if required.
