C. Facility Description
The above named permittee is hereby authorized to operate the facility shown on the application and approved drawings, plans, and other documents attached hereto or on file with the Department and made a part hereof and specifically described as follows:
• The CCAFS C&D Debris Disposal Facility (C&D Facility) is part of the CCAFS disposal site. The CCAFS disposal site also has an asbestos monofill and closed class I landfill. Both the asbestos monofill and closed class I landfill have separate permits, SO05-0026050-010 and SF05-0026050-008 respectively. The landfill property is approximately 182 acres with 172 acres of disposal area. To date, 67.5 acres have been used to dispose of asbestos containing materials, C&D, and class I waste.
• The class I landfill (Areas C-1 and C-2) was closed in 1985 with 24 inches of sediment, soil and grass. Areas C-1 and C-2 were filled with mixed MSW and C&D debris.
• The C&D Facility consists of 5 areas (Areas C-3, C-4 and C-5) and a scale house.
Expected volume of C&D to be received is approximately 90 tons/day. The C&D Facility does not have a bottom liner or a leachate collection system.
o Area C-3 received C&D debris from 1992 to 2005. An interim cover of sediment, soil and grass has been provided in this area. Area C-3 is considered inactive. C-3 is at approximately 14 feet NGVD with side slopes of 25:1.
o Area C-4 (8.17 acres) has been active since 2005. Cell 7 (0.37 acre) is the last next active cell of C-4. With DEP’s approval 10/18/2007, Vertical Integration Building (VIB) and Space Launch Complex 13 (CX13) debris were disposed in Area C-4.
o Area C-5 (36.36 acres) will be the future C&D debris disposal area. Cell 1
(0.32 acre) will be the first cell.
• A stormwater control system and permit are not required for the C&D Facility.
The in-situ soils are comprised of well drained sand mixed with shell fragments. Permeability is greater than 20 inches per hour. Stormwater has rarely been observed sheet flowing across the site; standing stormwater has never been observed.
• The Water Quality Monitoring Plan Implementation Schedule (MPIS) applies to the entire facility. The Plan attached to this permit replaces all previous MPIS.

WATER QUALITY MONITORING PLAN
IMPLEMENTATION SCHEDULE (MPIS)
(9/28/2011)
GENERAL
1. The CCAFS Disposal Site has an asbestos monofill, a closed Class I landfill, and a C&D disposal facility. Each type of facility has its own permit, but they share this one MPIS.
2. This Monitoring Plan is effective with the date of the permit. [62- 701.510(1)(b)&(c), 62-520.600(5), Florida Administrative Code (F.A.C.)]
3. The field testing, sample collection and preservation and laboratory testing, including quality control procedures, shall be in accordance with Chapter 62-160 (F.A.C.). Approved methods as published by the Department or as published in Standard Methods, ASTM, or EPA Methods shall be used. [62-701.510(2)(b), F.A.C.]
4. The organization collecting samples at this site must use the Field and
Laboratory Standard Operating Procedures (DEP-SOP-001/01) referenced in Chapter 62-160, F.A.C. Sampling personnel must have a copy of the SOP for purging and sampling in the field when sampling and must be knowledgeable of its contents, procedures, and forms. The laboratory designated to conduct the chemical analyses must be certified by the Florida Department of Health Environmental Laboratory Certification Program (DoH ELCP). This Certification must be for the test method and analyte(s) that are reported. [62-160.210(1), 62- 160.300(1), F.A.C.]
NOTE: DEP-SOP-001/01 can be accessed at:
http://www.dep.state.fl.us/water/sas/sop/sops.htm
5. If, at any time, analyses detect parameters which are significantly above background water quality, or which are at levels above the Department’s water quality standards or criteria specified in Chapter 62-520, F.A.C., in the detection wells or at the edge of the Zone of Discharge, the Permittee, to confirm the data, shall resample the wells within thirty (30) days of receipt of the sampling data.
Should the permittee choose not to resample, the Department will consider the water quality analysis as representative of current ground water conditions at the facility. The permittee must notify the Department within 14 days of receipt
