PERFORMANCE PLAN SUPPLEMENT
REQUIRING OFFICE: J2 – Joint Intelligence Operations Center - NORTH
TITLE: Maintenance and Repair of Electronic Security System
1. INTRODUCTION.
1.1 The purpose of this supplement is to provide the Quality Assurance Evaluator (QAE) a formal and effective assessment method for evaluating the contractor’s conformance to the requirements of this contract.
1.2 Assessment for this contractor will be based on the Service Delivery Summary (SDS) item/items identified in the Performance Work Statement. These have been included below with their Assessment Method and Frequency identified.

2. PERFORMANCE ASSESSMENT RESPONSIBILITIES.
| 2.1 | QAEs are to complete a Certificate of Service (COS) each month during contract performance. The COS will be provided to the Contracting Officer (CO) on the 10th business day of the month to account for the previous month’s performance. |
| 2.2 | QAEs may also receive feedback about the quality of contractor performance from the customer. All feedback will be noted and filed in the Performance Assessment Folder applicable to this contract. |
3. PERFORMANCE ASSESSMENT DOCUMENTATION.
3.1 Performance assessment documentation of contract performance will be maintained in a Performance Assessment Folder (PAF). The required sections of the PAF are as follows:
Section 1. Appointment Letters. This section must include:
a. QAE appointment letter and Phases 1 and 2 training certificates.
b. The CO’s Letter of Delegation to QAE
Section 2. Performance Plan
Section 3. Contract, Performance Work Statement and PWS Addendum (if applicable).
Section 4. Activity/Correspondence. Documentation of meetings or communication (hand-written or email) with the Contractor regarding task order performance. (All surveillance of contractor performance to include review of CDRLs and other deliverables needs to be documented in Section 4)
Section 5. Records. Documentation of all contractor performance assessment. This section shall contain completed Certificate of Service (COS) forms, completed Performance Evaluation Reports and any PARs issued to the Contractor.
3.2 The QAE will document communications with the Contractor concerning task order PWS requirements in Section 4 of the Performance Assessment Folder.
4. DEFICIENCY RESOLUTION.
The QAE must use his/her judgment in determining what type of action to take for a noted deficiency. An attempt should be made to resolve performance deficiencies at the lowest practical level. In the event any deficiencies are noted during performance of this contract, the QAEs are required to contact the Contracting Officer (CO) to determine actions that will be taken to effect corrective action.
