PERFORMANCE WORK
STATEMENT
FOR
Sewer Camera and Jetting
PREPARED BY:
66 CES/CEOES
March. 2018
1. DESCRIPTION OF SERVICES:
1.1. Scope: The contractor shall provide all labor, tools, equipment, hoses, gauges, test equipment, hearing protection, eye protection including prescription safety glasses, materials, supplies and parts, transportation and other incidentals necessary to travel to Hanscom AFB, MA, to perform maintenance on sewer systems. The contractor shall be responsible for insur-ing all services and parts provided under this contract are in accordance with the terms and conditions outlined herein and conform to the manufacturer’s equipment specifications and this Performance Work Statement (PWS). 1.2. Requested Service: The Contractor shall provide all tools, supplies, equipment, and la-bor necessary to perform maintenance of the sewer systems on Hanscom Air Force Base (AFB). Components include, but are not limited to: sanitary sewer lines and main connec-tions. Provide and execute a sanitary sewer cleaning and maintenance schedule for 62,115lf.
of sewer lines. Evaluate condition of sanitary sewer lines by first jetting the lines between manholes and then video camera the lines. Evaluate the structural condition of the piping and manholes, outlining items such as infiltration, root penetration etc. Clean and inspect com-plete manhole structure to include, but not limited to; all interior structure shelves, inverts, inlets, outlets and drops, noting any deficiencies. Submit a report to the Government Repre-sentative indicating findings including video and written report describing the conditions ob-served the report shall be submitted in electronic format.
1.2.1. Contractor shall vactor/jet identified sewer lines

1.2.3. Contractor shall provide flagmen to assist in the directing of base traffic. Ten day noti-fication needs to be provided to the Contracting Officer Representative (COR) to coordinate with base agencies.
1.2.4. Provide up-close imaging of pipe walls and surfaces during sewer line inspections of 4” relined pipe through 24” diameter pipe of sewer, storm, drainage and other pipe lines.
1.2.5. Contractor shall transport and dispose of removed material

1.3. Maintenance Plan for Areas Prone to Mechanical Failures: Provide a comprehensive list of problem areas based on active reviews of records and inspection by camera. The list should address the amount and type of debris in each section of line. Provide list of suggested corrective action which includes spot repairs, medium length sections and complete reaches between sewer manholes to include the manhole.
1.4. Maintenance Plan for Areas Prone to Silt and Grease deposits: identified on the GeoBase map are the areas that will require jetting more frequently. These areas identified consistently have silt, grease and other deposits. Ensure these areas are maintained/jetted and updated as required but at a minimum annually. Submit cleaning schedule and adjust schedule based on visual observations of pipe restrictions and deficiencies.
1.5. Maintenance Plan for Areas Prone to Root Penetration: In areas where jetting and/or camera indicate root penetration, remove of roots in the sanitary sewer system without the use of excavation. If pipes are in need of repair or replacement submit the location and infor-mation to the Government within 48 hours of observation. Suggest proper efficient method of repair/corrective action in the report. Execution of the repair is outside the scope of this con-tract.
1.6. Confined Space: This contract has the potential for entry into permitted and non-permitted confined space areas. Permit required confined space areas will require, IAW AFI 91-203, Chapter 23 The U.S. Air Force Mishap Prevention Program, and AFFARS Clause 5352.223-9001, Health and Safety on Government Installations, a prebrief prior to the start of any service which includes the contractor, COR, Safety Office and Fire Department. It is the contractor’s sole responsibility for compliance with OSHA (Public Law 91-956. The prebrief will inform the contractor of any known hazards and the contents of the space. Additionally, the contractor will be briefed on precautions and procedures implemented by 66 ABG/CE to protect Air Force workers. The Fire Chief will coordinate the confined space entry contract and either approves or disapproves use of the rescue team supplied by the installation FES Flight. At the conclusion of the contract, an after-action brief will be required where the con-tractor will provide information regarding hazards encountered or created in the space(s).The COR will contact the CO and ABG/SE immediately if an unsafe act or procedure is per-formed by the contractor.
1.7. Quality Assurance: According to the contract’s inspection clause, the government will evaluate the contractor’s performance under this contract. The COR is a government repre-sentative of the Contracting Officer (CO) and will participate in the administration of this contract. Any matter concerning changes to the scope, costs, terms or conditions of this con-tract shall be referred to the CO. All services to be performed by the contractor will be sub-ject to review by the CO or COR. The contractor shall provide an answer to a submitted Cor-rective Action Report (CAR) and Customer Complaint Form from the COR within one (1) working day from receipt. The contractor shall provide written response addressing the root cause, corrective action and preventive actions (s).
1.8. Continuation of Essential Services during Crisis: This requirement has been deter-mined not to be essential and does not require continued support during a crisis as define in DFARS Clause 252.237-7023.
1.9. Hours of Operation: The contractor shall perform work Monday thru Friday, under this contract during the following hours: 07:00 a.m. until 4:00 p.m., excluding Federal Holidays.
Work required to be performed outside of these hours shall only be done after approval from the contracting officer.
1.9.1. HOLIDAYS.

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled ser-vice day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.
1.10. Contractor Personnel: The contractor shall designate in writing the name of the Con-tract Manager (CM) and any alternate(s) who shall act for the contractor when the manager is absent and their telephone numbers where they may be contacted any time to the CO and the COR within five (5) calendar days after contract award. The CM or alternate shall have full authority to act for the contractor on all matters relating to operation of this contract at Hans-com AFB. The CM or alternate shall be available to meet on the installation with government personnel designated by the CO to discuss problem areas.
1.10.1. Listing of Contractor Employees: The contractor shall not employ persons on this contract if such employee is identified to the contractor as a potential threat to the health, safe-ty, security and general well being or operational mission of the installation and its popula-tion. The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activi-ties. The contractor shall be prohibited from employing off-duty COR’s who are surveillance any contracts/subcontracts awarded to the Contractor. All contract employees depending on particular condition, occasion or place, or job assignment shall require professional, suitable attire. All contractor employees shall conduct themselves in a professional, courteous man-ner. The selection, assignment, reassignment, transfer, supervision, management, and control of contractor employees in performance of this PWS shall be the responsibility and preroga-tive of the contractor; however, the contractor shall comply with the requirements set forth in this PWS and Hanscom AFB regulations concerning conduct of employees as referenced herein. The contractor shall employ and maintain a technically trained and experienced work force.
1.11. Environmental Protection and Safety: The contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and requirements regard-ing environmental protection and occupational safety and health. The contractor shall comply with any laws or regulation changes during the term of this contract. The contractor shall fol-low manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling (including hazardous materials).
1.12. Hazardous Material (HAZMAT): The contractor will comply with the Hanscom AFB. Hazardous Materials Operation Plan provided. The contractor shall not store chemi-cals/HAZMAT on Hanscom AFB. All chemicals/HAZMAT shall be approved prior to being transported on base for use by any contractor. The contractor shall prepare and submit to the COR, ten (10) calendar days after contract award date, one (1) each, Chemical/Hazardous Material Request Authorization Form (Air Force Form 3952) and Safety Data Sheets (SDS) for each chemical, material and substance proposed to be used in performance of this PWS.
The COR will forward the completed AF Form 3952(s) and SDS to 66 ABG/CEIE for ap-proval. The contractor shall not transport or use any chemical, material or substance on Hanscom AFB before written approval is provided by 66 ABG/CEIE. Contractor shall main-tain copies of SDS(s) for all chemicals. The SDS(s) shall correspond with the manufacturers of the chemicals on hand. Manufacturer, material, and MSDS shall match. NOTE: SDS re-fers to manufacturer MSDS. Once material is approved for use, contractor shall contact base Environmental Management (EM) (66 ABG/CEIE) and provide a list of all material. 66 ABG/CEIE shall determine any additional material requirements based on the hazard code of the material.
1.13. Hazardous Waste: Contractor shall dispose of any spent/processed chemical solu-tions and used rags in waste drums located in the shop/area where the work is being per-formed.
1.14. Discharges: Any discharges into industrial waste treatment plant (IWTP) or sanitary sewer shall be done in accordance with Hanscom Industrial Waste Water Discharge Permit as instructed by 66 ABG/CEIE. The contractor shall maintain records of written authorization issued by 66 ABG/CEIE for discharges into IWTP and sanitary sewer.
1.15. Spills: Upon occurrence of any spills that could enter the storm system or cause any harmful environment effects, the contractor shall immediately call 911 to report the incident.
When possible, the contractor shall use a base telephone to receive help from Hanscom AFB
911. If using a cell phone, request Hanscom AFB 911. The contractor shall report all envi-ronmental violations to the COR. The contractor shall submit a typed or written report about the incident within four (4) clock hours. The contractor shall provide five (5) copies of the written report. The contractor shall provide one copy to the CO, two copies to the COR, a copy for Unit Environment Coordinator (UEC) and a copy for record. The format of the re-port shall be left up to the contractor, but the report shall contain the following information.
1.) Name and contact number of person reporting the spill 2.) Date and time of incident 3.) Location and source of spill 4.) Substance or pollutant spilled 5.) Amount spilled and the rate of discharge 6.) Any damages or injuries involved 7.) Extent of the area impacted 8.) Potential hazards 9.) Action taken 10.) Organizations contacted 11.) Remarks Section - that addresses the cause of the spill (e.g. human error, Equipment) and whether 911 was called and which agencies responded.
1.16. Environmental and Safety Training: The contractor personnel shall have Resource Conservation Recovery Act (RCRA) environmental and safety training required by the state and federal government. The training shall be IAW the following regulations: Resource Con-servation Recovery Act (RCRA) 40 Code of Federal Regulation (CFR) 265.16 “Personal Training”, Occupational Safety and Health Administration (OSHA) Hazardous Waste Opera-tions and Emergency Response Standard Title 29, CFR Part 1910.120 “Hazardous Waste Op-erations and Emergency Reponses”, Hazardous Waste Management Instruction, Section 13, and Air Force Instruction (AFI) 32-7042, “Solid and Hazardous Waste Compliance”, Section 2, The contractor shall provide the environmental and safety training certificate for all em-ployees to the CO and COR, after contract award date.
1.17. Housekeeping: The contractor shall keep the work areas clean and neat in accordance with Industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this PWS.
1.18. Tool Control and Accountability: The contractor or contractor’s personnel shall in-ventory the container or bag of tools at the beginning of each work day and at the end of each work day. Contractor personnel shall immediately notify the COR of any tools that are miss-ing or lost from the work area or container (i.e. tool or job box or tool bag). The COR will report to appropriate government personnel of any missing contractor’s tool(s) not located within one hour after notification of lost or missing tools.
1.19. Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Hanscom AFB per AFI 31-204 (in its entire-ty), Motor Vehicle Traffic supervision.
1.20. Visitor Pass: The contractor shall comply with requirement and procedures IAW FAR 5353.242-9000, Contractor Access to Air Force Installations. A list of names will be required to gain access to the installation Passes shall be distributed at the visitor’s center at the Van-denberg gate. Workers shall have proof of identification as well as a valid license, registra-tion and insurance if bringing a vehicle onto base. Contractor employees must have identifi-cation readily identifies them as contractors. During the course of the contract, certain are-as/sites may be restricted or closed to contractor personnel. The contractor shall comply with all requirements for entry. In addition, unscheduled gate closures may occur at any time and personnel entering or exiting the installation may experience delays. Once gaining access on-to Hanscom AFB, Contractor shall immediately sign the logs at Building 1810, indicating time and date of arrival. Before departure, Contractor shall again sign the logs indicating the departure time, as well as the work performed and the buildings worked in. A copy of a signed work receipt on Contractor’s letterhead detailing work performed shall also be given to the Civil Engineering Government Representative or left in the log book prior to departure.
1.21. Foreign National(s): The contractor shall notify the COR before sending a Foreign National representative to perform services. Special rules apply for foreign national(s) visit-ing a US Government facility.
1.22. Health and Safety on Government Installation: In performing work under this con-tract on a Government installation, the contractor shall comply with federal, state and local regulations regarding occupational safety and health and the specific health and safety re-quirements established by this contract. The contractor shall notify the COR of any injuries that occur to contractor personnel while on the installation. The contractor shall notify the COR of damage to government property or equipment during the execution of the contract.
1.23. Emergency Procedures: Contractor employees shall follow the direction of Govern-ment employees in regards to emergency evacuation procedures, i.e. fire, tornado, active shooter and bomb threats. It is the contractors’ responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with government personnel in all fire and tornado drills and at no additional cost to the government.
SECTION 2
Performance Assessment
The contractor shall be aware that in the absence of any contract requirement from the Service Summary does not detract from its enforceability nor limit the rights or remedies of the gov-ernment under any other provision of the contract. The contractor shall ensure the following service summary requirements are accomplished.

3.1. Government Furnished Property (GFP): Government Furnished Property is not ap-plicable to the in the performance of this contract.
3.2. Reserved.
3.3. Government Furnished Services: Government Furnished Services are applicable dur-ing the performance of this contract.
3.3.1. Security Police: The government will provide general on base Security Forces ser-vice. The Security Forces’ telephone number is 911 for emergencies, for crime in progress 781-225-5613 and 781-225-5613 for non-emergency calls.
3.3.2. Fire and Emergency Services: The government will provide general on-base fire protection services. The contractor shall comply with fire prevention rules and regulations enforced by the Hanscom AFB Fire Prevention Branch. Call 911 for emergencies. For rou-tine fire prevention matters and/or calls to dispatch, call 781-225-3330.
3.3.3. Emergency Medical Service: Calling 911 from a cell phone will be received by off base emergency 911 systems, causing a delay in response time by emergency personnel.
When possible use a base telephone to receive on base help.
3.3.4. Utilities: The government will furnish utility services. These utilities include natural gas, electricity, water and sewer for the sole purpose of accomplishing this contract. If the contractor requires additional utility services, it shall be the contractor’s responsibility to pro-vide. Contractor shall use due care and diligence in efforts to conserve utilities to reduce util-ity costs.
3.3.5. Refuse Collection: The government will provide refuse collection services limited to the extent provided in the Base Refuse Collection Contract for government provided facilities.
The government will only allow non-hazardous refuse to be emptied into the nearest author-ized refuse dumpster for trash collection. Do not use the government dumpsters for collection of construction debris.
3.3.6. Workspace: The government will provide adequate workspace around the equip-ment area.
