About this file
This is a Price Schedule exhibit (Exhibit D) for a Food Service Request for Proposal issued by the State of Minnesota for the Rochester Community Behavioral Health and Health (CBHH) location under contract release number F-515. The solicitation seeks a food service provider to support the Minnesota Direct Care and Treatment facility. Prospective responders must submit proposals through the Supplier Portal by the deadline specified in the Event Details, with all questions regarding the solicitation due in writing to the designated buyer by Friday, July 31st, 2026, at 3:00 PM Central Time. Responses must include comprehensive documentation including responder declarations, exceptions to state terms and conditions, workforce certifications, equal pay certificates, and evidence of required credentials such as a Certified Food Protection Manager certificate, Registered Dietician credential verification, vendor license, Department of Health inspection results, and dietary manager's certificate.
The Price Schedule exhibit establishes the pricing structure and financial terms for food service delivery at the Rochester CBHH facility. Responders are required to provide detailed pricing information in accordance with the format and specifications outlined in this exhibit as part of their formal proposal submission. The contract award will be contingent upon the State's evaluation of pricing competitiveness alongside technical qualifications, compliance documentation, and the responder's ability to meet all operational and regulatory requirements for food service provision in a behavioral health treatment setting.
