About this file

This is a Description of Intended Single Source Purchase (PUR 7776) form filed by the Florida Department of Health, Division of Medical Quality Assurance. The agency seeks to purchase licensure revenue services, data warehousing, and support services for the Axiom Pro solution integrated with the Licensing and Enforcement Information Database System (LEIDS). The procurement covers a three-year contract term and requires ongoing integration with multiple applications to support processing, deposits, and loading of applications, renewals, and miscellaneous documents. The posting was made on October 2, 2026, with the deadline for receipt of information set for October 23, 2026, at 5:00 PM. The contact for this procurement is Yasemin Sulker at the Florida Department of Health, located at 4052 Bald Cypress Way, Tallahassee, FL 32399, and can be reached at 850-245-4931 or via email at Yasemin.Sulker@flhealth.gov.

The estimated dollar amount for this single-source acquisition is $3,216,300.48. Docufree Corporation has been identified as the intended single source vendor, as the company owns the Axiom Pro Solution and is the only entity authorized to provide the requested services and maintain system functionality for this proprietary system. The justification for single-source procurement emphasizes that continued use of the Axiom Pro and LEIDS systems is necessary to maintain compatibility and continuity of the integrated systems. The purchase was approved by Sonja German-Jones, Purchasing Director. Prospective vendors are invited to provide information regarding their ability to supply the described commodities or services; however, the agency has determined that these services are available only from a single source in accordance with Florida Administrative Code Rule 60A-1.045 and Florida Statutes Section 120.57(3).
