About this file
This is a Description of Intended Single Source Purchase (Form PUR 7776) from the Florida Department of Health (DOH) Bureau of Chronic Disease Prevention for a Pilot Project on Epilepsy and Tobacco Free Florida Services Program. The DOH seeks to purchase tobacco education and referral services for Florida residents with epilepsy and their caretakers. The scope includes updating the organization's website with Tobacco Free Florida information, referring at least one resident monthly to local Area Health Education Centers for smoking cessation, conducting presentations on epilepsy and tobacco prevention, performing tobacco use screening assessments, providing tobacco cessation training, and establishing EHR-based referrals. The contract term is one year.
The intended single source provider is Epilepsy Services of West Central Florida (doing business as Florida Epilepsy Services) with an estimated contract value of $90,000. The justification for single-source acquisition is that Florida Epilepsy Services is one of only five organizations in the established network of epilepsy providers partnered with the Department's Epilepsy Services Program, and it is the only provider authorized to serve Hillsborough, Highland, Hardee, and Polk counties under section 385.207(2)(a), Florida Statutes. The purchase is being processed in accordance with Chapter 60A-1.045 Florida Administrative Code, with a 15-business day posting period for other vendors to submit information about their ability to provide these services.
