About this file

This document is a Description of Intended Single Source Purchase (PUR 7776) issued by the Florida Department of Health (DOH), Division of Emergency Preparedness and Community Support, Bureau of Preparedness and Response. The DOH has a need to purchase annual maintenance services for its custom mobile medical facility, including warehouse storage, inspection, preventative maintenance, repairs, and component replacement to ensure the continued operability and functionality of the unit, which serves as a mobile hospital with isolation room for deployment during disasters or incidents. The requested services are for a 3-year term, with a prospective vendor response deadline of April 18, 2024, at 3:00 PM.

The DOH has identified FORTS Logistics LLC as the sole source provider for these services, as the manufacturer's warranty stipulates that FORTS Logistics LLC is the only service provider authorized to perform maintenance, repairs, and component replacement on the mobile unit, and allowing any other vendor to do so would void the warranty. The estimated dollar amount for the 3-year contract is $316,500. The single-source justification is based on the unique and necessary nature of the services required and the manufacturer's sole-source warranty requirement.
