About this file
This document is a Description of Intended Single Source Purchase (PUR 7776) posted by the Florida Department of Health (FDOH), Correctional Medical Authority (CMA). The CMA requires the purchase of a three-year subscription to TrackVia, a third-party business application that integrates with the CMA's current electronic data collection platform, Formstack Forms. The CMA is required by Florida Statute to conduct triennial surveys of the Florida Department of Corrections health care delivery system, and is transitioning to electronic records keeping for these audits. The TrackVia platform is needed to facilitate data analysis and reporting capabilities that the CMA's current system does not provide.
The estimated dollar amount for this single-source acquisition is $64,400. The justification is that TrackVia's platform is the only available integration through the CMA's Formstack Forms data collection platform that meets the required data analysis and reporting needs. The FDOH, CMA is requesting authorization to purchase the TrackVia subscription as a single-source contract in accordance with Florida Administrative Code. Prospective vendors are invited to provide information about their ability to supply the requested services by July 18, 2023.
