About this file
This document comprises evaluation and maintenance forms for the Montana Department of Transportation's truck parking area maintenance contract at Bozeman Hill, I-90 Milepost 326.5. The file includes three exhibits detailing daily safety checklists, contract compliance review procedures, and comprehensive maintenance task requirements. The contract scope encompasses cleaning and maintenance of facilities including vault toilets, parking lots, sidewalks, garbage containers, landscaping, building exteriors, storage sheds, and associated grounds. Daily maintenance activities are required across all facility components, with specific performance standards established for cleanliness, appearance, safety, and operational functionality. The contractor must maintain daily logs documenting all completed activities and report unusual occurrences, which are submitted to the Department twice monthly for compliance review.
The contract compliance review process utilizes a four-tier rating system (0-3) with categories for unacceptable, needs improvement, contract minimums, and exceeds minimums performance. Inspections are conducted jointly by Department representatives and contractor personnel, with ratings documented and signed by both parties. The evaluation criteria encompass facility cleanliness, structural maintenance, equipment functionality, employee appearance and identification requirements, and record-keeping compliance. Payment authorization requires approval from the Maintenance Chief and is contingent upon satisfactory performance ratings during the inspection period. The contractor must ensure all cleaning materials are properly stored with current Safety Data Sheets (MSDS), maintain operational fire extinguishers and first aid kits, and employ staff wearing identification badges at all times during operations.
