About this file
This document is an Exhibit A - Sample Contract for a Purchased Service Contract between the Washington State Department of Social and Health Services (DSHS) and a Contractor. The contract outlines the requirements for the contractor to provide DVD translation and subtitling services for DSHS training DVDs. The contract term is not specified, but the contractor will be required to provide translated scripts with independent review prior to DVD creation, as well as deliver the final master DVDs for DSHS review and approval. DSHS will review the DVDs within 10 business days and identify any issues for correction by the contractor. The total maximum consideration payable to the contractor is up to an unspecified amount, to be based on the contractor's proposal and acceptance by DSHS.
The contract includes detailed general and special terms and conditions, including requirements for contractor insurance, billing and payment procedures, and a dispute resolution process. It also incorporates by reference the original RFQQ 1342-002 issued by DSHS and the contractor's response, which likely contain additional details on the specific services, quantities, and timeline for this opportunity.
