Requirements Traceability Matrix Summary

This is an Exhibit N – Requirements Traceability Matrix for Request for Offer (RFO) No. HHS0015558v2, issued by the Texas Department of State Health Services (DSHS) for a Video Directly Observed Therapy (VDOT) application supporting tuberculosis medication adherence monitoring. The contract encompasses three distinct phases: Transition, Operations, and Turnover. The Transition Phase occurs between the Contract Effective Date and Operations Start Date, requiring the contractor to complete knowledge transfer from the incumbent provider, develop transition and readiness assessment plans, train all providers on both mobile and web-based applications, migrate client data, and achieve full system functionality within specified timeframes. The Operations Phase commences on the Operations Start Date and continues through contract expiration, during which the contractor must provide 24-hour online technical support via chat and email, plus telephone support during business hours (7:30 a.m. to 5:30 p.m. CST, Monday-Friday). The application must support both synchronous and asynchronous observation modes, function on web-based and mobile platforms (iOS and Android), provide multilingual support in English and Spanish, include comprehensive reporting and analytics dashboards, maintain HIPAA compliance with end-to-end encryption, and achieve TX-RAMP Level 2 certification. The Turnover Phase begins 180 calendar days prior to contract expiration or ten business days after early termination, requiring the contractor to ensure uninterrupted service continuity while transferring operations, data, and intellectual property to DSHS or a successor provider.

The contract establishes nine key deliverables with specific due dates, including a Transition Plan (seven business days after contract effective date), Weekly Written Status Reports (commencing five business days after contract effective date and continuing weekly throughout the contract term), Readiness Assessment Plan and Results Report, Turnover Plan (due 180 business days after operations start date with annual updates), System Security Plan (within twenty-one calendar days of contract effective date), and Root Cause Analysis and Corrective Action Plans as requested by DSHS. Key Performance Measures include minimum 99% monthly uptime, timely notification of unanticipated downtime within one business hour, deliverable submission deadlines with liquidated damages of $200 per additional business day (not to exceed $2,000 monthly), and Key Personnel replacement within ten business days at a rate of $200 per business day (not to exceed $4,000 monthly). The contractor must maintain all resources in the contiguous United States, employ staff with appropriate HIPAA training, designate an Account Manager with minimum two years of contract management experience in healthcare, and assign a Technical Lead with minimum five years of application architecture management experience. The application must comply with Web Content Accessibility Guidelines (WCAG) 2.1 Levels A and AA, provide SOC2 Type II annual attestation, and support integration with the State's Identity and Access Management (IAM) multi-factor authentication system.

3. Deliverables For each Deliverable, DSHS may require a walk-through of the Deliverable in advance of delivery. DSHS will provide ten (10) Business Days advanced notice to Contractor when a Deliverable walk-through is required.

B. Deliverable Acceptance

2. Each Deliverable must be reviewed and approved by DSHS prior to its use. Approval will be granted if DSHS determines the Deliverable conforms to the accepted specifications identified in the Contract. DSHS acceptance of any Deliverable does not relieve the Contractor of the liability or responsibility for corrections for subsequent defects, errors, and/or omissions discovered or experienced in connection with the Deliverable.

3. If any Deliverable cannot be provided within the scheduled timeframe, the Contractor must contact DSHS’s point of contact, in writing, and provide a reason for the delay and the proposed revised schedule, which DSHS may accept or reject at its sole discretion. DSHS’s acceptance of a Deliverable after the applicable due date does not relieve the Contractor from its obligation to submit a complete Deliverable by the specified due date.

4. DSHS shall notify the Contractor in writing of Deliverable approval or rejection, along with the reason(s) for rejection and the steps the Contractor must complete so the Deliverable will be acceptable.

5. Upon receipt of the corrected and resubmitted Deliverable, DSHS shall review the corrected Deliverable to confirm that the identified deficiencies have been corrected and brought into compliance with specifications and that the corrections have not directly or indirectly caused other deficiencies. DSHS shall have the option to continue the evaluation process until the Deliverable is brought into compliance with the Contract specifications.

6. Any Deliverable that is not explicitly accepted by DSHS is rejected.

C. Deliverable Due Dates

All deliverables requested by DSHS must be provided on the dates specified in Exhibit N, Requirements Traceability Matrix, (Section 3 (Deliverables)). Failure to provide deliverables on the due dates may result in the assessment of liquidated damages as listed in Exhibit N, Requirements Traceability Matrix, (Section 2 (Key Performance Measures and Liquidated Damages)).
