About this file
Exhibit N is a Past Performance Questionnaire used to evaluate bidders' qualifications and relevant experience for a federal lease opportunity. The questionnaire requires prospective lessors to disclose whether they have experience with federal lease projects and, if so, the number of federal leases successfully completed. Bidders must provide detailed information on previous federal lease experience, including the number of leases managed, duration and terms of each lease, scope of services provided (such as maintenance and janitorial services), the federal agencies involved, and contact references for each lease with names, titles, phone numbers, and email addresses. If a company has no past performance history relating to leases, the questionnaire allows for submission of details on relevant contracts previously performed or completed to establish qualifications.
This questionnaire is part of Solicitation 36C25626R0038 for a Request for Lease Proposal (RLP) regarding a new or replacement Vet Center (#741) facility in Mobile, Alabama, issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 16. The evaluation of past performance through this exhibit serves as a critical component in assessing bidder capability to manage and deliver the required leasing and facility services for this federal healthcare facility.
