About this file

This document is an Exhibit M Program Performance Report template for the Arizona Department of Economic Security (DES) Emergency Solutions Grant (ESG) Program. The report is designed for quarterly and annual tracking across multiple Continuum of Care (CoC) regions, including Maricopa, Tucson, and Balance of State (BOS). It provides a standardized reporting mechanism for homeless service providers to document program performance metrics related to various homeless assistance activities.

The document outlines performance reporting requirements for four primary ESG-funded activities: Emergency Shelter, Street Outreach, Rapid Re-housing, and Homeless Prevention. The performance metrics include key indicators such as persons served, service rates, bed utilization, income exit rates, destination outcomes, data quality, length of participation, and grant expenditure percentages. The report template requires providers to calculate and report these metrics quarterly and annually, with a focus on tracking progress, identifying barriers, and assessing solutions for each homeless service program. The goal appears to be comprehensive performance monitoring and continuous improvement of homeless assistance services across Arizona's different CoC regions.
