About this file
This document is Exhibit J from a state contract opportunity for the Promoting Safe and Stable Families (PSSF) Program, specifying mandatory data fields required for entry into the CDEC Salesforce Database by participating agencies. The exhibit outlines the demographic information and case management data that must be collected and entered into the system, organized by step and entry type. Required fields are designated with red asterisks and include household account information such as primary caregiver name, phone number, county, and family composition; contact records for primary caregivers and children including first name, last name, gender, birthdate, race/ethnicity, Hispanic/Latinx status, tribal member status, and primary spoken language; and PSSF client case details including case start date, record type, status, household, site, county, and supervisor assignment. Additional mandatory documentation includes a Data System Consent Form completed before family information entry, CFSA 2.0 Assessments for families receiving intensive case management or meeting specific service thresholds, and service activity records documenting service type, start date, service category, individual receiving service, and related activities.
The document also requires documentation of goal setting activities with goal names and topics, optional resource referral and flex funding entries, and case closure procedures with status changes and closure reasons. While many fields are mandatory, best practice guidance recommends completing as much information as possible to ensure comprehensive case documentation. The fields and their mandatory designations may change with system updates, and all information entered must be verified as accurate and complete. No pricing, funding amounts, contract terms, set-asides, or award information is addressed in this exhibit, as it functions as a technical specification document for data management requirements rather than a procurement instrument.
