About this file

This is an Exhibit document detailing the Intensive Case Management Guidelines for the Promoting Safe and Stable Families (PSSF) program, applicable to awarded service sites. The guidelines establish comprehensive requirements for implementing intensive case management services to eligible families, with case managers maintaining caseloads of 7-10 families for service durations typically spanning 3-12 months. The program targets kinship families, families in post-permanency stabilization, families in crisis with multiple identified needs, and those with or without active child welfare involvement. Core service components include initial intake procedures requiring minimum face-to-face meetings and data system entry, collaborative family engagement models such as Family Group Decision Making and Individualized Service and Support Teams meetings, standardized assessments using CFSA2 and Protective Factors Survey tools, and family-driven goal setting with clear milestones and measurable outcomes documented in Salesforce.

Contact standards require minimum weekly face-to-face interventions during months one through three, with frequency adjusted to family needs thereafter, with all activities documented in the OEC data system. Additional services include optional emergency support and concrete needs assistance (allowable up to 15 percent of total PSSF award), social connection facilitation through ecomaps and community activities, resource navigation and advocacy through multi-system planning, and transition services with celebration of success at case closure. Case management supervision must include clinical oversight, routine consultation, quality monitoring, and documented case reviews. Quality assurance and improvement activities require regular file reviews, collection of family satisfaction surveys, and submission of mid-year and year-end program reports with supervision and case review timeframes specified in the Statement of Work.
