About this file
This is a Proposer's Disclosure Statement form issued by the Board of Regents of the University System of Georgia on behalf of Kennesaw State University for Project No. 555-27-058, Design, Construction Standards, and Technical Specifications Update. The disclosure document requires all proposers submitting qualifications or proposals to complete a conflict of interest assessment prior to submittal. The form requires proposers to disclose any business transactions with the Board of Regents, the using agency, or the project's end-user occurring within the prior two years; any gifts, hospitality, or benefits provided to these entities within the prior one-year period; and any conflicts of interest or potential conflicts of interest as defined by Georgia law and Executive Orders that could result in private monetary or financial benefit or detriment to the proposer, the proposer's relatives, or any associated business entities. The disclosure must be dated and signed by an authorized representative of the proposing firm and submitted as a required deliverable with the proposal.
The Proposer's Disclosure Statement serves as a compliance mechanism to ensure transparency and prevent the appearance of impropriety in the selection and contracting process for this public project. All proposers are required to complete this form regardless of their background or prior relationships with the university or state agencies. The statement emphasizes that the Board of Regents will make every effort to avoid conflicts of interest through this mandatory disclosure process, and submission of the completed and signed disclosure is a prerequisite for proposal acceptance.
