About this file

This document is an Income Verification Calculation Worksheet created by the Arizona Department of Economic Security (DES) for use in the Rapid Rehousing (RRH) program serving youth and families. It comprises two primary components: a Declaration of No Income form and an RRH Income Verification Worksheet designed to establish and document client financial eligibility for program assistance.

The worksheet establishes income eligibility requirements based on HUD income limits, requiring household income to be at or below 50% of Area Median Income (AMI) for the applicable jurisdiction at the time of verification. Income certification must be completed after financial assistance has been provided for one year. The document requires calculation of total annual income from all sources including employment, government benefits, child support, spousal support, disability, retirement, assets, and self-employment income using the HUD CPD Income Calculator. Subrecipients must document verification through either third-party income documentation or client self-certification forms. The Declaration of No Income component requires clients claiming zero income to complete detailed questionnaires and provide written verification from income sources confirming that resources used for expenses are not derived from the client's own income. The worksheet references the HUD income calculator accessible through hudexchange.info and requires that income calculations be printed and maintained in client files for documentation purposes.
