Exhibit J Shelter Diversion Quarterly Performance Report

This document is a quarterly performance reporting template issued by the Department of Economic Security (DES) State Homeless Coordination Office for shelter diversion program contractors. The template is designed for state and local agencies providing shelter diversion services to report on their activities, outcomes, and progress across four quarters within a contract period extending to June 30, 2026. The report structure accommodates multiple quarterly submissions (Q1: July-September, Q2: October-December, Q3: January-March, and Q4: April-June) and requires submission by designated agency representatives with accompanying contact information and submission dates.

The reporting requirements encompass three primary sections: Section A collects comprehensive demographic data on all program participants including participant totals, age breakdowns, race/ethnicity, and additional demographic categories such as LGBTQ identification, disability status, language service needs, and youth experiencing dating violence. Section B requires documentation of households served through unduplicated counts, average assistance amounts per household, and categorical breakdown of assistance types including rent/utility assistance, move-in costs, childcare support, legal fees, employment-related expenses, and other diversion-related services. Section B also requires tracking of success metrics including housing maintenance rates, emergency shelter utilization within 3-6 months post-assistance, and quarterly housing stability rates. Section C mandates narrative responses addressing significant diversion activities, participant success stories, contract-enabled services, community partnerships, participation in evaluation processes, and emerging community trends or unmet needs. The template contains no pricing information, as it functions as a performance tracking and outcomes measurement tool rather than a procurement or pricing document.
