About this file

This document appears to be an Exhibit E - Frequency Chart that is part of a state or local contract opportunity. The Frequency Chart outlines the locations, frequency, and types of pest control services required for various areas of a school building, including the exterior, cafeteria, kitchen, food storage, custodial closets, and other common areas. The chart specifies the pests to be monitored and/or controlled, such as rodents, insects, and birds, and the methods to be used, including trap glue boards, bait boxes, and pest checkers. The chart also indicates that the current work order system, FMX, is used to manage and assign pest control work orders based on priority and urgency.

The document provides details on the service requirements and frequencies, but does not include information on pricing, set-asides, incumbents, funding sources, or contract term and renewals.

* Customer requests reporting pest issues are received daily from APS building sites. These requests are filtered through an APS work order system. The current work order system we use is called FMX. FMX manages and assigns work orders specifically concerning pest control for contractor review and scheduling based on the priority and urgency of the requests.
Please reference sections 3.24 and 3.25 for FMX work order requests and scheduling
