About this file

This is a Rate Sheet submission document for Agreement 26VS0002, a Boiler Maintenance and Incidental Repairs contract issued by the California Department of Veterans Affairs. The contract requires the contractor to provide all labor, materials, staff, transportation, licenses, permits, and related expenses necessary to deliver boiler inspection, maintenance, and incidental repair services at the Home facility upon request and in compliance with agreement terms. Services are divided into five primary categories: semi-annual services (4 visits over the contract term), annual services (2 visits over the contract term), non-emergency repairs during business hours (Monday-Friday, 8:00 AM-5:00 PM), emergency services during business hours (Monday-Friday, 8:00 AM-5:00 PM), and emergency repairs outside business hours and on weekends and holidays. The contract term spans two years, and submission of the completed rate sheet is mandatory for bid consideration.

Pricing for the contract is structured on a per-visit and hourly basis across service categories, with bidders required to specify rates for semi-annual services, annual services, non-emergency repairs (estimated 40 hours), and emergency services (estimated 20 hours each for business and non-business hours). Additionally, bidders must provide a fixed price of $20,000.00 for replacement parts for two boilers. The total contract value, which serves as the basis of award, combines all line items (A through G). The State explicitly reserves the right to modify the actual amount of services required based on facility needs and makes no guarantee regarding service volume. Rates quoted by the successful bidder become binding for the entire two-year contract term. All bids must include a signed rate sheet transmitted via email in accordance with solicitation instructions, and failure to indicate a dollar amount for any line item will be interpreted as the bidder offering that service at no cost.
