About this file

This is a Price Proposal Form for a Personal Duress Alarms project in Colorado involving equipment, installation, monitoring, and maintenance services. The project requires vendors to supply ATLAS-based locating appliances and transmitters manufactured by Actall, with specific quantities including one Standard Hubsens Locating Appliance server, 300 Heavy Duty Transmitters, 152 ceiling-mount locators, 12 outdoor enclosure locators, one low-profile ceiling mount locator, and 23 Power Over Ethernet gateways. The proposal form requests pricing for project services including initial design, software programming, integration support, surveys, Level 3 installation inspections, certification, and end-user training with all travel costs included. The contract structure includes a base year of agreement followed by four optional renewal years, each with associated monitoring and maintenance rates.

Pricing must be submitted in five sections: Actall equipment costs, installation costs including necessary cabling, conduit, network infrastructure, and one-year installation and manufacturer warranty, and monitoring and maintenance services for the base year and each optional year. The monitoring and maintenance section requires vendors to specify monthly costs per alarm for monitoring services, monthly costs per alarm for maintenance services, hourly rates for service calls during business and after-hours, and hourly rates for removal, reinstallation, and relocation services during business and after-hours for all five contract years. Vendors must coordinate with Actall to obtain equipment costs and are required to complete ATLAS training prior to installation and Level 1/Level 2 testing before scheduling Level 3 inspection and final project commissioning.
