Exhibit A Statement of Work: Duress Alarm Upgrades and Services
This is a Statement of Work for duress alarm system upgrades and services for the State of Colorado. The scope of work encompasses the upgrade, installation, and ongoing maintenance of duress alarm systems across specified state facilities and locations. The project involves assessment of existing duress alarm infrastructure, design of upgraded systems, procurement of necessary equipment and components, and implementation of new or enhanced duress alarm capabilities. The work includes both initial installation phases and long-term service and monitoring commitments to ensure continuous operational capability of duress alarm systems. The statement of work establishes the technical requirements, performance standards, and service level expectations that contractors must meet throughout the contract term, with specific attention to response times, system reliability, and integration with existing security infrastructure.
The contract requires comprehensive documentation of all system upgrades, testing protocols, and maintenance schedules. Contractors must demonstrate expertise in duress alarm system design and installation, with qualifications validated through industry certifications and prior experience with similar government security installations. Service provisions include 24/7 monitoring capabilities, rapid response protocols for system failures or false alarms, and regular preventive maintenance to maintain system integrity. The work may involve coordination with multiple state agencies or facilities, requiring clear communication protocols and documentation procedures. Contract performance is measured against specific technical standards and response time requirements, with provisions for system audits and compliance verification throughout the contract term.

Statement of Work: Personal Duress Alarm (PDA) System Project Title: Implementation and Operation of the Personal Duress Alarm System at Hawkins High-Security Building
1. Introduction and Purpose The primary goal of this Statement of Work is to define the requirements, functions, and expected outcomes for the Personal Duress Alarm system which needs to be replaced and/or upgraded. The equipment provider for this project is Actall Corporation (Actall equipment list is part of the Exhibit B Pricelist and this approximation vendor should verify the equipment: The equipment shall be included in the bid amount) based on its ability to utilize and integrate with the existing PDA infrastructure currently deployed at the facility. The preferred vendor will be an authorized Actall Corporation supplier/install company and capable of installing the Actall equipment to complete this project as described in this SOW. The system is designed to establish a high-functioning, technology-driven platform that ensures immediate emergency notification, accurate location identification, and comprehensive support for all life safety and security emergencies within the designated facility. The system is essential for safeguarding the welfare of all patients and the safety of facility staff.
2. Project Goal To provide a reliable, real-time duress signaling and communication platform critical for emergency notification/response and staff/patient safety within the Hawkins high-security environment; improve staff and patient safety outcomes; support post-incident investigation, reporting, and continuous quality improvement with the use of Actall equipment included in the cost.
2.1 Applicability and Population Served
The PDA system is mandated for use within the Hawkins high-security building. This service is critical to the safety of up to 229 patients and all assigned staff members.
3. Scope of Work and Mandatory Requirements This SOW encompasses the full lifecycle of the Personal Duress Alarm system, including operation, infrastructure maintenance, and adherence to mandatory usage protocols, configuration and commissioning, training, and documentation.
3.1 Staff Requirements (Mandatory Use)
3.1.1 Required Wear: All staff working in the Hawkins building are required by policy to wear a personal duress alarm device for the full duration of their scheduled shifts.
3.1.2 Training: System use and functionality instructions are mandatory components of all new employee orientation and continuous staff education programs.
The vendor shall provide training materials and support as defined in Section 7.
3.2 System Operational Scope
The scope includes the provision and maintenance of a dedicated infrastructure to ensure real-time communication during activation.
4. System Functionality and Emergency Response The PDA system must operate using dedicated, high-availability infrastructure to meet the critical life safety requirements of the facility.
4.1 Infrastructure Requirements
Dedicated Infrastructure: The system shall operate using dedicated servers and utility software to maintain consistently high availability and reliability, independent of general facility networks.
4.2 Key System Functions
Function
Description
Direct Alert Communication
Immediate notification to the Public Safety Communications Center upon confirmed duress activation.
Campus-Wide Notification
Capability to issue rapid alerts to designated response groups across the campus.
Real-Time Location Verification
Accurate, validated location identification using Actall Corp. (or equivalent) lightbars located directly on the Unit(s)
Redundancy and Failover System
Redundancy to prevent single points of failure
Audit and event logging
Comprehensive logging of all activations, acknowledgements, and responses.
4.3 Data Capture Requirements
The system must capture and securely store comprehensive, time-stamped data essential for incident management and analysis. This data includes, but is not limited to:
· Exact time of duress activation.
· Specific, validated location of the activation point.
· Verified response times by supporting personnel (CMHHIP and others).
· System health and availability metrics.
5. Integration with Post-Incident Processes The data provided by the PDA system will serve as the foundation for the facility's post-incident quality management and investigation processes.
5.1 Incident Report Generation
Following a duress activation and resolution, the captured PDA data will be used to generate a mandatory incident report via the RLDatix system.
5.2 Investigation and Follow-Up Flow
The generation of the RLDatix report must automatically trigger immediate investigation and follow-up activities involving the following internal teams:
| 5.2.1 | The Quality Management occurrence and investigations team. |
| 5.2.2 | Nursing and operational leadership. |
| 5.2.3 | Administrative review (as deemed necessary by leadership). |
6. Service Outcomes and Metrics (Benefits) The established processes and reliable data supported by the PDA system are designed to achieve measurable improvements and support continuous quality management.
6.1 Measured Outcomes
Outcome
Metric / Benefit
Improved Response Times
Utilization of verified response time data to identify performance bottlenecks and implement procedural enhancements.
Facilitation of Safer Outcomes
Ensuring a rapid, coordinated, and location-specific response to maximize safety for patients and staff during critical incidents.
Enhanced Follow-up and Training
Providing essential, objective data for analysis within critical incident reports, which directly informs targeted staff training and policy adjustments.
Contribution to Event Reduction
Leveraging trend data and post-incident analysis to identify systemic risks and implement preventative measures, with the ultimate goal of decreasing the frequency of future duress events.
7. Training and Documentation The vendor shall provide:
· Administrator-level training:
· End-user training materials;
· System documentation, including operating manuals and troubleshooting guides All documentation shall be delivered in electronic format.
8. Acceptance Criteria System acceptance shall be contingent upon:
· Successful completion of functional testing;
· Verification of location accuracy;
· Successful activation and notification testing;
· Delivery of all required documentation and training.
Acceptance shall be documented in writing by the Facility.
