About this file
This document is an Agency Long Term Space Requirements Needs Interview Questionnaire completed by the VA Phoenix Health Care System (PVAHCS) for a new clinical space lease initiative.
The VA seeks to lease approximately 9,281 usable square feet of space to support PVAHCS initiatives, with a focus on offering timely, quality services for Veterans. The project encompasses multiple clinical and administrative functions, including Physical Medicine and Rehabilitation services, Mental Health patient care, Eye Clinic/Optical services, Community Based Outpatient Clinic (PACT), and Pathology and Laboratory Medicine services. The lease term is 10 years with a 5-year firm term for tenant improvement (TI) cost recovery. The agency currently has 37 staff members and requires 70 parking spaces for privately owned vehicles within 0.1 miles of the building. The delineated area is bounded by W. Mountain View Boulevard, W RH Johnson Boulevard, West Greenway Road, and AZ State Route 303 in a non-rural location with proximity to public transportation. Weekday operations run 6:30 AM to 5:30 PM with after-hours access required for janitorial and maintenance services. The facility must meet Security Level II requirements with intrusion detection, access control systems, sealed IT room walls extending to overhead structure, and dedicated locks. GSA assistance is needed for IT/Network and Telecommunication Services, furniture acquisition, office equipment, security products, and move coordination. The agency will participate in Design Intent Drawing (DID) workshops with key decision-makers including the Chief Engineer, Space Planning, Service Chiefs, and Contracting Officer Representative. Circulation is accounted for in the square footage request, and new furniture will be purchased rather than moved.

DID Workshops DID workshops are 1-3 day meetings held after award hosted by the Lessor with their architect. DID workshops also provide for a more collaborative design process resulting in a design that will better meet the needs of the agency which increases overall satisfaction with the space. In order for this process to be successful, the agency must bring all of the “decision makers” to the meeting and be prepared to approve the DID.
