About this file
This specification document outlines the detailed requirements for Blood and Urine Collection Kits for the State of Oregon. The urine collection kits must contain a 90ml specimen container with temperature indicator strip, specimen label, biohazard transport pouch, tamper evident tape, procedural instructions, and a mailing box approximately 5" x 3" x 2" with shrink-wrapped contents. The urine kit box must feature centered green-bordered labels at least 1" x 2.5" with green ink printing of "Urine Collection Kit," reorder number, and lot number. Blood collection kits must include a 21-gauge safety needle (BD Vacutainer Eclipse #368607 pre-approved), single-use needle holder (BD Vacutainer #364815 pre-approved), two 10ml evacuated glass tubes with gray closure containing 10mg Sodium Fluoride and 20mg Potassium Oxalate (BD Vacutainer #367001 pre-approved), blood collection report, non-alcoholic preparation pad, tamper evident tape, biohazard transport pouch, and a comparable mailing box designed to protect and secure glass tubes during transport using polystyrene foam, cardboard cutouts, or bubble envelopes.
Blood collection kit boxes must feature centered red-bordered labels at least 1" x 2.5" with red ink printing of "Blood Collection Kit," reorder number, expiration date of blood collection tubes, and lot number. Both kit types must be designed with single openings that do not tip easily when stacked. Procedural instructions for sample collection will be provided by the agency. The specifications emphasize biohazard safety measures, proper labeling and evidence documentation, secure packaging for transport integrity, and use of pre-approved medical supplies to ensure compliance with collection standards and bloodborne pathogen exposure prevention protocols.
