BLK 4 – The report will be in the contractor’s format and include receipt of any Government Furnished Information or Government Furnished Property in the report. The report should include personnel changes, invoicing, data collection dates, and any issues with the contract progress.
BLK 9 - The following information shall be included on the deliverable: DISTRIBUTION STATEMENT D: Distribution authorized to the Department of Defense and U.S. DoD contractors only, Administrative or Operational Use, January 2023. Other requests shall be referred to Commander, Marine Corps Systems Command (Attn: PMM-140.7), 2200 Lester Street, Quantico, Virginia 22134.
BLK 12 – Submit after first 30 days following contract award and then monthly thereafter.
BLK 13 – Submit monthly.
BLK 14 – A draft copy will be submitted to the Contract Officer Representative (COR) five (5) business days prior monthly due date to allow for Government review, concurrence, and response.
BLK 14 – Government response to the draft report is required within three (3) business days of receipt. Any non-response by the Government will constitute Government concurrence with draft report.
BLK 14 – A final copy of the report, incorporating any Government revisions from the draft will be submitted to the COR with the Contract Specialist carbon copied (Cc)

BLK 12 – Submit draft survey plan in contractor’s format to the COR within 30 days of contract award. The draft plan will then be used during discussions in order to develop the final anthropometry survey plan. The final plan will be delivered five days after survey plan discussions have been completed and agreement on the measurements and methodology to be completed are finalized. All submissions are electronic in MS Word format. Final plan format if changed from contractor format will be clarified during discussions of the draft plan.
BLK 13 – Submit updated survey plans as required based on agreed changes or change requirements identified through discovery.
BLK 14 - Submit all final copies to the COR, with the Contract Specialist Cc.

BLK 9 – Deliverable is a standard working form and does not require distribution marking.
Format will be provided from the USMC Human Research Protection Program Policies and Procedures dated 30 Sept 2020.
BLK 12 – Submit CITI Certificates and Investigator Affirmations within 30 days of Contract Award. If personnel changes occur, the additional personnel will require CITI certificates and Investigator Affirmation prior to participating in data collection and analysis. The CITI training site is provided below and is free training. The selected organization for affiliation for CITI is Department of the Navy.

BLK 4 – The Anthropometry Survey Training Handbook and Materials will be provided in Contractor format and should complement the 2010 Anthropometric Survey of U.S. Marine Corps Personnel measurement methods. The training will include the measures identified in the USMC Anthropometry Survey Plan. The training will ensure Government personnel are capable of collecting anthropometry data if required. The training also ensures Government personnel are knowledgeable of the methodology and equipment used in the survey process. The training material will be updated if the data collection methodology or survey plan changes.
BLK 9 - The following information shall be included on the deliverable: DISTRIBUTION STATEMENT D: Distribution authorized to the Department of Defense and U.S. DoD contractors only, Administrative or Operational Use, January 2023. BLK 12 – Draft training material will be delivered to the COR (30) days prior training being conducted to allow for Government review and comment. The Government review will be completed and returned to the contractor for proper adjudication (15) days after receipt of draft material. A final training handbook draft will be provided to the COR (30) days prior to completion of the period of performance for Government review and returned to the Contractor within (15) days. The final handbook will be provided as a final deliverable at contract completion in conjunction with the final report. The Government will provide reproduction capability for the student copies of all materials on paper.
BLK 14 - Submit a copy of the USMC Anthropometry Survey Training Handbook and Training Materials to the COR and notify the contracting specialist.

BLK 4 – The Digital Database will be provided in a format that meets the Government requirements. The database will meet ISO 15535 requirements identified unless otherwise changed by the Government. The data field formats of the data base will be finalized no later than the first year of the contract once enough data has been collected to make informed decisions on the format or potential beneficial adjustments due to discovery during the survey process.

BLK 12 – The final digital database will be delivered in a digital format on an external hard drive and also on a Government provided computer hard drive (30) days prior to contract completion.
