About this file
This document is an Initial Deficiency Report (IDR) template issued by the District of Columbia Government's Department of General Services for the Turnkey District-Wide Conveyance System Inspection, Testing, Maintenance and Break/Fix Services contract (DCAM-27-NC-IFB-0008). The template provides a standardized format for qualified District-licensed elevator contractors to document the baseline condition assessment of elevators, escalators, lifts, and other conveyance equipment located at various District-owned and/or operated facilities. The IDR captures comprehensive facility and unit information, operational condition summaries, code compliance and safety assessments, mechanical and electrical systems evaluations, maintenance and reliability assessments, and deficiency identification with categorization by severity and priority levels.
The IDR template requires contractors to certify that all assessment findings were completed using accepted industry methodologies, that all conditions and deficiencies have been fully and accurately documented based on direct physical inspection, and that no deficiencies have been omitted, minimized, reclassified, or altered. The report must identify specific deficiencies with descriptions, components affected, severity levels, code references, safety and operational impacts, recommended corrective actions, priority assignments, estimated costs, and associated supporting documentation. Additionally, the template addresses modernization needs with rationale tied to ASME, NEC, OEM, or lifecycle requirements. The completed IDR is submitted to the Contracting Officer and Contracting Officer's Technical Representative in Excel (unlocked) or Excel and searchable PDF formats, either via email or through District-designated platforms such as Salesforce.
