About this file
This document is an instructional form from the California Department of Transportation (Caltrans) Local Assistance Procedures Manual designed for local agencies to report the utilization of Disadvantaged Business Enterprises (DBEs) and first-tier subcontractors on state and locally-funded contracts. The form, Exhibit 17-F, serves as a final report to be completed upon contract completion and submitted to both the local agency and Caltrans District Local Assistance Engineer. The report requires documentation of all first-tier subcontractors and DBEs regardless of tier status, including their certification numbers, contract payment amounts, work completion dates, and final payment dates. Contractors and consultants must certify the accuracy of reported information, while local agency representatives must verify that contracting records and on-site DBE performance were monitored throughout contract execution.
The form captures critical contract information including the local agency contract number, federal-aid project number, contractor name and business address, final contract amount, and a detailed breakdown of work items and services supplied. Payment reporting distinguishes between DBE and non-DBE entities, with specific counting rules applied based on whether materials are obtained from DBE manufacturers (100% credit) or DBE dealers/suppliers (60% credit). The document includes comparative tracking of original DBE commitment amounts versus actual utilization, requiring explanation of any variances. Completion requires signatures and contact information from both the contractor/consultant representative and an authorized local agency representative, establishing accountability for accurate reporting of DBE participation on publicly-funded transportation and infrastructure projects.
