About this file
This is a Past Performance Questionnaire (PPQ) template that contractors must complete and distribute to their clients for evaluation as part of the NIST Shuttle Services solicitation (1333ND26QNB190079). The form requires contractors to gather feedback from two distinct client points of contact: a Technical POC and a Contracting POC. Each reference must complete their respective section and return the form via email to Helen Wesley (helen.wesley@nist.gov) and Tony Schmidt (tony.schmidt@nist.gov). The document is limited to three pages in length.
The PPQ captures company-specific information and project details, including the client organization name, contract/task order number, brief description of support provided, period of performance with dates and costs, and number of contractor personnel by skill category. The Technical POC section evaluates overall technical performance, quality of work, schedule performance, cost performance, risk identification and mitigation capabilities, and willingness to engage the contractor again, with ratings ranging from Excellent to Poor. The Contracting POC section assesses overall contractual performance and likelihood of future engagement. Both sections require contact dates and the name, title, address, and telephone number of the respective points of contact. The form serves as a standardized mechanism for NIST to evaluate contractor past performance during the proposal evaluation process.
