About this file
This document is the Evaluation Tool for the Request For Application (RFA) for the Massachusetts Department of Mental Health's (DMH) Group Transportation Master Agreement. The RFA is seeking applicants that can provide transportation services to transport groups of DMH patients/clients and staff from one location to another in non-medical emergency and non-emergency situations. The Evaluation Tool outlines the minimum submission requirements, review criteria, and evaluation process for the applicants. It includes details on the required application components, applicant qualifications, vehicle and driver requirements, and quality control and safety standards. The RFA is seeking applicants that have the capacity to transport a minimum of 48 adult individuals at one time, at least 3 years of experience providing safe and dependable charter transportation services, and are in sound fiscal condition. Qualified applicants will be placed on a Master Agreement list, with DMH areas and communities they can serve noted.
The Cost Response Workbook portion of the application will be evaluated to ensure the rates are reasonable and competitive. Applicants that are Small Business Purchasing Program participants may be given preference. The evaluation will identify any strengths, weaknesses, and points for negotiations. Successful applicants will be awarded a contract with an unspecified term, likely with renewal options.
