The form is mandatory for all respondents and requires comprehensive disclosure of potential conflicts of interest across multiple categories. Respondents must submit a completed and signed form as part of their solicitation response; failure to provide complete responses and proper documentation may result in disqualification or contract termination. The form requires disclosure in five primary areas: ownership structure and major stakeholders (individuals with greater than 10% ownership or pecuniary interests valued at $25,000 or more), paid consultants with contracts valued at $25,000 or more, interested parties with controlling or intermediary interests, subcontractors and suppliers with their contact information and contract values, and gifts or benefits offered to TxDMV officials during the preceding two years. Additionally, respondents must disclose all communications with TxDMV senior officials within the past calendar year and one year preceding solicitation advertisement, as well as any participation in solicitation development or preparation of specifications.
