About this file

This is an Execution of Offer document from The University of Texas System for an RFQ for Fire and Life Safety Code Inspections in Texas. The document serves as the formal certification and agreement form that respondents must complete, sign, and submit with their qualifications to be considered for contract award. Respondents must acknowledge that the RFQ is a solicitation for qualifications only and does not guarantee contract award, that they will bear all costs associated with preparing their response, and that they agree to furnish the products and/or services described in their qualifications while complying with all terms, conditions, and requirements outlined in the RFQ documents.

The document requires respondents to make numerous certifications and representations, including confirmation that they are a reputable company with necessary experience and resources to perform the required services, that all submitted information is current, complete, true, and accurate, and that they maintain compliance with all applicable federal, state, and local laws. Additional certifications address Texas franchise tax obligations, antitrust law compliance, maintenance of required insurance, Equal Employment Opportunity and Affirmative Action compliance, OSHA safety standards compliance, and disclosure of any financial interests held by University of Texas System Board members or executive officers. Respondents must provide their State of Texas Tax Account Number, identify all owners holding at least 25 percent of the business entity, certify they have no relationships with UT System employees within the prior twelve months, and confirm that no compensation was received for participating in specification preparation. Failure to complete, sign, and return this form will result in disqualification of the submittal.
