About this file
This is Addendum #2 to Invitation for Bid (IFB) 2026-B14 for the Firecracker and Main Street Traffic Signal project issued by Hays County Purchasing Office in San Marcos, Texas. The addendum was released on August 19, 2026, and requires vendors to acknowledge receipt and return the signed addendum with their proposals. Failure to submit the acknowledged addendum may result in the proposal being deemed non-responsive and removed from consideration. The addendum includes three required forms that must be completed and submitted with all bid packages: a Vendor Reference Form requesting three current customer references with similar size and scope of work, a Conflict of Interest Disclosure and Questionnaire (Form CIQ) as required by Chapter 176 of the Texas Local Government Code, and a Related Party Disclosure Form.
The Vendor Reference Form requires vendors to provide three references with detailed company information, contact persons, phone numbers, email addresses, and descriptions of contract scope and duration. The Conflict of Interest Questionnaire must be filed with Hays County's records administrator no later than the seventh business day after a vendor becomes aware of facts requiring disclosure, with violations constituting a Class C misdemeanor. The Related Party Disclosure Form mandates transparency regarding relationships between the vendor and current or former Hays County employees within the second degree of consanguinity or affinity. Vendors must certify that all information provided is true and complete, and submission of a complete proposal package without these forms will render the proposal non-responsive and ineligible for award. All questions regarding this addendum were answered by Hays County staff, and inquiries should be directed to Stephanie Hunt, CPPB, Purchasing Agent, at purchasing.bids@hayscountytx.gov or 512-393-2267.
