About this file

This is a Conflict of Interest Disclosure and Certification form (Exhibit D) for RFP No. 301-26-06531, issued by the State of Texas for Foreign Trade and Investment Services related to the Texas Israel Office. The form is a mandatory execution document that must be completed, signed, and submitted with the respondent's proposal to avoid disqualification. Respondents are required to certify that they have read and understand the conflict of interest provisions outlined in RFP Part B, including Sections 14, 16, 21, and 54, and to confirm that neither they, their employees, nor their subcontractors possess any interests that would create actual, potential, or apparent conflicts of interest or adversely impact contract performance.

The disclosure requires respondents to either itemize any actual or potential conflicts of interest or certify that no such conflicts exist. Additionally, respondents must certify compliance with three specific sections of the Texas Government Code: Section 669.003 (restricting contracts with executive heads of state agencies), Section 572.069 (prohibiting employment of state officers and employees involved in the procurement), and Section 2252.901 (addressing contracts with former or retired agency employees). If any of these statutes apply, respondents must provide the name of the former executive or employee, the state agency involved, the separation date, and the employment date with the respondent. Respondents also acknowledge a continuing duty to supplement disclosures if circumstances change or additional information becomes known after proposal submission, requiring prompt submission of updated information to the procuring agency.
