About this file
This is a Texas Family Code Schedule certification document for Solicitation No. 601440000053578, issued by the State of Texas. The schedule serves as an eligibility verification form that respondents must complete and return with their proposal submissions. Respondents are required to certify their eligibility to receive state grants, loans, or contract payments under Texas Family Code Section 231.006, which addresses child support obligations. The document mandates that any response must include the names and Social Security numbers of all individuals or business entities holding at least twenty-five percent ownership interest in the submitting business entity. Failure to sign and return the completed schedule may result in the response being deemed non-responsive and ineligible for consideration.
The schedule requires respondents to provide ownership information in a structured format with fields for names and Social Security numbers of qualifying owners. Respondents who do not meet the ownership criteria threshold may check a box indicating the schedule is not applicable and proceed accordingly. The document includes a Federal Privacy Act Notice explaining that Social Security number disclosure is mandatory under Texas Family Code Sections 231.006(c) and 231.302(c)(2) and that this information will be used to identify individuals who may owe child support obligations. The state commits to maintaining confidentiality of Social Security numbers to the fullest extent permitted by law. Respondents must sign and date the certification, providing their name and title, with this executed page required as part of the proposal submission package.
