About this file
This is a Texas Family Code Schedule document for Solicitation No. 601440000053567, issued by the State of Texas. The schedule serves as a compliance certification requirement for all respondents submitting proposals in response to this state contract opportunity. Respondents must complete the schedule by providing ownership information for any individual or business entity holding at least twenty-five percent ownership interest in the submitting organization, including names and Social Security numbers for each owner meeting this threshold. The completed and signed schedule must be returned with the response submission, as failure to do so may render the response non-responsive.
Under Texas Family Code Section 231.006, respondents certify their eligibility to receive state payment and acknowledge that the contract may be terminated and payment withheld if the certification is inaccurate. The document includes a Federal Privacy Act Notice informing respondents that disclosure of Social Security numbers is required by Texas Family Code Sections 231.006(c) and 231.302(c)(2) for purposes of identifying individuals who may owe child support obligations. The Social Security numbers provided will be kept confidential to the fullest extent permitted under Section 231.302(e) of the Texas Family Code. Respondents who do not meet the ownership criteria may certify that the schedule is not applicable by checking the designated box, signing, and returning the form.
