About this file
This document is a Vendor Supplemental and Special Conditions form issued by the Texas Department of Family and Protective Services (DFPS) that outlines contractual obligations and remedies applicable to vendors providing services under DFPS contracts. The form, designated as N502 FORM-5622V (June 2022), indicates that there are no supplemental conditions modifying the DFPS Vendor Uniform Terms and Conditions, establishing a baseline for all contractor compliance requirements under this contract opportunity.
The special conditions section establishes three primary remedies available to DFPS in the event of contractor noncompliance or poor performance. First, DFPS may issue a Corrective Action Plan (CAP) identifying areas of deficiency, requiring the contractor to respond in writing within specified timeframes, address each identified defect, and submit responses for DFPS review and approval before implementing the corrective measures. Second, DFPS reserves the right to suspend or remove all or any portion of the contract. Third, DFPS retains the authority to require the removal of any employee, volunteer, agent of the contractor, or subcontractor from service provision or to prohibit such personnel from direct contact with DFPS-referred clients or client records. These conditions establish performance accountability standards and give DFPS significant oversight authority regarding contractor staffing and service delivery compliance.
