About this file
This is a Texas Family Code Schedule document for solicitation number 601340000064266, administered by the State of Texas. The schedule serves as a compliance certification requirement for all respondents bidding on state contracts. Respondents must provide accurate information regarding child support obligations and ownership interests in their business entities. The document requires submission of names and Social Security numbers for any individual or business entity holding at least twenty-five percent ownership interest in the responding organization. Completion and return of this signed schedule is mandatory and constitutes a material requirement of the bid submission process.
Under Texas Family Code Section 231.006, respondents certify their eligibility to receive state payments and acknowledge that the contract may be terminated and payment withheld if the certification is inaccurate. The Social Security numbers collected are used to identify persons who may owe child support obligations and are kept confidential to the fullest extent permitted by law under Section 231.302(e) of the Texas Family Code. Failure to sign and return this schedule with the response may result in the response being deemed non-responsive and potentially disqualify the bid from consideration.
