About this file
This is a Form 1 Execution of Response document for the Texas Workforce Commission (TWC) Workforce Diploma Pilot Program solicitation (RFP Number 3202600137). The form serves as the official execution page that respondents must complete, sign, and submit to acknowledge receipt of all TWC terms and conditions outlined in the RFP, including Attachment 1, and to agree to provide the items or services described in the solicitation. Respondents must indicate whether they are taking exceptions to any terms and conditions specified in the RFP documentation. Any exceptions must be identified in detail and submitted with the response; exceptions not specifically identified at the time of submission will not be considered. The form requires respondents to affirm that they meet the Minimum Qualifications identified in Section 5.2 of the RFP and that they have obtained the necessary federal Employer Identification Number (EIN) and Texas Identification Number prior to contract award.
The form collects comprehensive respondent information including the organization's legal name, mailing address, billing address, phone number, fax number, email address, and tax identification numbers. It also requires designation of a point of contact for proposal submission and a separate point of contact for contract administration. Respondents must disclose any financial interests in proposed subcontractors that may perform work under an executed contract. The form includes notification requirements for respondents to inform TWC of substantial changes in financial condition, ownership or control, and any pending litigation, investigations, or adverse actions prior to or during contract performance. Additionally, the form provides checkboxes for respondents to claim preferences under Texas Administrative Code 34 TAC 20.38, which includes various categories such as Texas resident service-disabled veteran ownership, Texas-produced goods and services, recycled materials, energy-efficient products, and other designated preference categories.
